摘要
目的:探究慢性病健康管理模式在社区居家养老服务中的应用效果。方法:选取2018年10月-2019年7月社区慢性病患者110例,随机分为两组,各55例。对照组给予常规管理模式;研究组给予慢性病健康管理模式。比较两组患者管理效果、管理满意度及生活质量评分。结果:研究组管理总有效率、管理满意度及生活质量各项评分均高于对照组,差异有统计学意义(P<0.05)。结论:慢性病健康管理模式的效果明显,患者病情得到有效控制,临床症状明显改善,管理满意度提高,生活质量提升。
Objective:To explore the application effect of chronic health management mode in community home care service.Methods:From October 2018 to July 2019,110 patients with chronic disease in the community were selected and randomly divided into two groups with 55 cases in each group.The control group was given conventional management mode.The research group was given chronic health management mode.The nursing effect,nursing satisfaction and quality of life scores were compared between the two groups.Results:The total management effective rate,management satisfaction and quality of life scores in the research group were higher than the control group,with the differences were statistically significant(P<0.05).Conclusion:The effect of chronic health management mode is remarkable,the patient's condition is effectively controlled,the clinical symptoms are significantly improved,the management satisfaction is improved,and the quality of life is improved.
