阿奇霉素在铜绿假单胞菌小鼠感染中的作用机制及对炎症因子的影响研究 被引量:2

摘要 目的探讨阿奇霉素在铜绿假单胞菌小鼠感染中的作用机制及对炎症因子的影响。方法选择BALB/c小鼠30只,平均分为三组,分别为研究组、对照组及空白对照组。研究组和对照组采用行气道接种的含有假铜绿单胞菌的琼脂糖对其进行感染。感染成功后,对照组应用0.9%氯化钠注射液进行治疗,研究组应用小剂量的阿奇霉素进行治疗。治疗后第1天、第3天、第7天对三组小鼠分别行支气管肺泡灌洗,比较三组小鼠治疗前后的炎症因子白介素-6(IL-6)、白介素-17(IL-17)水平以及TLR4 mRNA表达水平。结果空白对照组小鼠无肺部感染的症状发生,对照组小鼠与研究组小鼠在感染假铜绿单胞菌均后出现打喷嚏、呛咳、精神状态差等临床表现。在治疗7 d后,研究组小鼠呼吸急促等临床表现有所缓解,肺组织病理学炎症细胞浸润明显减少。对照组小鼠治疗后生理及病理皆无明显改善。感染7 d后,与空白对照组比较,研究组和对照组小鼠细菌培养量、肺泡灌洗液中白细胞计数、IL-6和IL-17水平、TLR-4 mRNA表达水平均明显升高(t分别=43.78、43.10、8.17、9.04、6.71、5.71、8.42、8.61、15.70、13.57,P均<0.05)。研究组小鼠和对照组小鼠细菌培养量、肺泡灌洗液中白细胞计数、IL-6和IL-17水平、TLR-4 mRNA表达水平之间比较,差异均无统计学意义(t分别=0.21、0.00、0.05、0.02、0.05,P均>0.05)。治疗7 d后,研究组白细胞计数明显低于对照组(t=4.40,P<0.05)。治疗3 d和7 d后,与对照组比较,研究组小鼠的IL-6、IL-17水平和TLR-4 mRNA的表达水平明显降低(t分别=3.81、6.26、7.19、5.09、18.21、15.88,P均<0.05)。IL-6、IL-17与TLR-4 mRNA的表达呈正相关(r分别=1.67、1.34,P均<0.05)。结论阿奇霉素在铜绿假单胞菌小鼠感染中的抗炎作用较好,可能是通过下调TLR-4表达水平实现的。 Objective To investigate the mechanism of azithromycin in pseudomonas aeruginosa infection in mice and its effect on inflammatory factors.Methods Thirty BALB/c mice were randomly divided into three groups:study group,control group and blank control group.The study group and control group were infected with agarose containing pseudomo-nas aeruginosa inoculated into the airways.Once the infection was successful,the control group was treated with normal sa-line,and the study group was treated with low dose azithromycin.The levels of IL-6,IL-17 and the expression of TLR4 mRNA were compared between the three groups of mice treated with bronchoalveolar lavage 1 day,3 day and 7 days after treatment.Results There was no symptom of pulmonary infection in the blank control group.After infection with pseudo-monas aeruginosa,the control group and the study group showed sneezing,coughing,poor mental state and other clini-cal manifestations.After 7 days of treatment,the clinical manifestations such as shortness of breath in the study group were relieved,and the infiltration of inflammatory cells in lung tissue pathology was significantly reduced.After 7 days of infection,compared with the blank control group,the amount of bacterial culture,the count of white blood cell in BALF,the levels of IL-6 and IL-17,the expression levels of TLR-4 mRNA in the study group and the control group were significantly in-creased(t=43.78,43.10,8.17,9.04,6.71,5.71,8.42,8.61,15.70,13.57,P<0.05).However,there were no significant dif-ference between the study group and the control group(t=0.21,0.00,0.05,0.02,0.05,P>0.05).After 7 days of treat-ment,the count of white blood cell in the study group was significantly lower than that of the control group(t=4.40,P<0.05).Compared with the control group,the levels of IL-6,IL-17,and the expression of TLR-4 mRNA in the study group were significantly decreased after 3 days and 7 days of treatment(t=3.81,6.26,7.19,5.09,18.21,15.88,P<0.05).IL-6 and IL-17 were positively correlated with TLR-4 mRNA expression(r=1.67,1.34,P<0.05).Conclusion The an-tiinflammatory effect of azithromycin is better in mice infected with pseudomonas aeruginosa,possibly through down regulating the expression level of TLR-4.
