芪参益气联合芪苈强心对心力衰竭患者呋塞米应用剂量及血肌酐的影响 被引量:3

Effects of Qishenyiqi combined with Qiliqiangxin on the dosage and safety of furosemide as well as on serum creatinine in patients with heart failure

摘要 目的比较心力衰竭患者24 h尿量、血肌酐及呋塞米应用剂量在入院当天、入院第4天及第7天的变化,探讨芪参益气联合芪苈强心对心力衰竭患者呋塞米应用剂量及血肌酐的影响。方法选择2019年12月至2020年5月收治的心功能(NYHA)Ⅲ、Ⅳ级患者90例,随机分为常规组和治疗组,每组45例。常规组应用常规西药规范治疗,治疗组在常规组基础上加用芪参益气和芪苈强心,检测入院当天、第4天、第7天患者血肌酐水平、24 h尿量、观察呋塞米应用剂量。结果治疗组总有效率为94.0%明显高于对照组的72.0%,差异有统计学意义(P<0.05)。常规组第1天、第4天和第7天尿量差异无统计学意义(P>0.05);治疗组第4天和第7天尿量明显多于第1天,第7天尿量明显多于第4天,差异有统计学意义(P<0.05);治疗组第1天、第4天和第7天尿量明显多于常规组,差异有统计学意义(P<0.05)。第1天,2组呋塞米的应用剂量均为40 mg;第4天,常规组患者呋塞米的应用剂量为60 mg,治疗组患者呋塞米的应用剂量为30 mg;第7天,常规组患者呋塞米的应用剂量为80 mg,而治疗组患者呋塞米的应用剂量为20 mg。常规组呋塞米的应用剂量呈上升趋势,治疗组呋塞米的应用剂量呈明显下降趋势。结论芪参益气联合芪苈强心不仅可使心力衰竭患者临床症状得到改善,还能使呋塞米应用剂量明显减少,一定程度上保护了患者的肾功能,值得临床推广应用。 Objective To investigate the effects of Qishenyiqi combined with Qiliqiangxin on the dosage and safety of furosemide as well as on serum creatinine in patients with heart failure.Methods A total of 90 patients with NYHA(heart functionⅢ,Ⅳgrade)were randomly divided into two groups,with 45 cases in each group.The patients in control group were given conventional treatment,however,the patients in observation group,on the basis of control group,were treated by Qishenyiqi combined with Qiliqiangxin.The serum levels of creatinine,24-hour urine volume and the application dose of furosemide on admission(1d),and at 4d,7d after treatment were observed and compared between the two groups.Results The total effective rate in observation group was 94.0%,which was significantly higher than that(72.0%)in control group(P<0.05).The 24-hour hour urine volumes at 1d,4d,7d after treatment in observation group were significantly higher than those in control group(P<0.05),however,the serum levels of creatinine at 1d,4d,7d after treatment in observation group were significantly lower than those in control group(P<0.05).On the first day,the application dose of furosemide in both groups was 40 mg;On the 4th day,the application dose of furosemide was 60mg in the routine group and 30mg in the treatment group;On the 7th day,the application dose of furosemide was 80mg in the routine group and 20mg in the treatment group.The application dose of furosemide in the routine group showed an upward trend,and the application dose of furosemide in the treatment group showed a significant downward trend.Conclusion Qishenyiqi combined with Qiliqiangxin can not only improve the clinical symptoms of patients with heart failure,but also can reduce the dosage of furosemide,which protects the renal function of patients to a certain extent.
