摘要
目的比较糖化清蛋白(GA)和果糖胺(FRU)变化值与血糖变化值的相关性,为2型糖尿病病人短期血糖控制选择更好的评价指标。方法将84例住院2型糖尿病病人按糖化血红蛋白(HbA1c)水平分为2组,分别于入院第2天及出院前1 d时检测GA、FRU、HbA1c、空腹血糖(FPG)、餐后2 h血糖(2hPG)和指尖血糖谱(包括三餐前、三餐后2 h、3:00共7个时间点)。计算指尖血糖谱均值(MBG),以及出入院GA值下降幅度(△GA)、FRU值下降幅度(△FRU)、平均血糖下降幅度(△MBG)、FPG下降幅度(△FPG)、2hPG下降幅度(△2hPG)。分析2型糖尿病病人△GA、△FRU与△FPG、△2hPG、△MBG的相关性。结果2型糖尿病病人△FPG、△2hPG、△MBG与△GA存在相关性(r=0.247~0.406,P<0.05),其中以△FPG与△GA相关性最好;2型糖尿病病人△FPG、△2hPG、△MBG与△FRU无明显相关性(r=0.131~0.184,P>0.05)。结论GA在反映住院2型糖尿病病人短期血糖变化上优于FRU,可以用于评估降糖方案调整后的疗效,指导2型糖尿病病人的临床诊疗。
Objective To investigate the correlation of the changes in glycated albumin(GA)and fructosamine(FRU)with blood glucose fluctuation,and to choose better indicators for evaluating short-term blood glucose control in patients with type 2 diabetes mellitus.Methods A total of 84 T2DM inpatients were enrolled and divided into two groups according to HbA1c le-vel.GA,FRU,HbA1c,fasting blood glucose(FPG),2-hour postprandial glucose(2hPG),and fingertip blood glucose spectrum(at 7 time points,i.e.,before three meals,at 2 hours after three meals,and at 3:00)were measured on day 2 after admission and on the day before discharge.Mean blood glucose(MBG)was calculated,as well as the reductions in GA(△GA),FRU(△FRU),MBG(△MBG),FPG(△FPG),and 2hPG(△2hPG).The correlation of△GA and△FPG with△FPG,△2hPG,and△MBG in T2DM patients was analyzed.Results△PG,△2hPG,and△MBG were correlated with△GA in T2DM patients(r=0.247-0.406,P<0.05),and△FPG showed the most significant correlation with△GA,while△FPG,△2hPG,and△MBG had no significant correlation with△FRU in T2DM patients(r=0.131-0.184,P>0.05).Conclusion GA is superior to FRU in reflecting short-term blood glucose fluctuation in hospitalized patients with T2DM,and therefore,it can be used to evaluate the effect of adjusted hypoglycemic regimen and guide the clinical diagnosis and treatment of T2DM patients.
