摘要
目的探讨海南地区汉族新诊断T2DM患者临床特征。方法回顾性分析2012年5月至2019年5月于解放军总医院海南医院内分泌科住院治疗的汉族新诊断T2DM患者326例,根据籍贯分为海南本地(海南组,n=173)、秦岭-淮河以北(北方组,n=112)和秦岭-淮河以南(南方组,n=41)共3组,比较各组一般资料、胰岛功能、DM并发症及合并症等。结果海南组年龄高于南方组(P<0.05),胰岛β细胞功能指数低于北方组(P<0.05)。海南组BMI、DM家族史阳性率及血肌酐低于南方组和北方组(P<0.05或P<0.01),中学及以下文化程度比例、HbA1c、TC、LDL-C及糖尿病周围神经病变(DPN)发生率高于南方组和北方组(P<0.05或P<0.01)。多元线性回归分析显示,文化程度、DM家族史、LDL-C是海南组HbA1c的影响因素。结论海南地区汉族新诊断T2DM患者文化程度、家族史阳性率及BMI较低,血糖水平高,胰岛功能差,DPN发生率高。
Objective To summarize the clinical characteristics of hospitalized patients with newly diagnosed type 2 diabetes(T2 DM)in Han Chinese population in Hainan.Methods A retrospective study was conducted in 326 Han patients with newly diagnosed T2 DM admitted to Hainan Hospital of Chinese PLA General Hospital from May 2012 to May 2019.They were divided into Hainan local group(Hainan group,n=173),north of Qinling Huaihe River group(North group,n=112)and south of Qinling Huaihe River group(South group,n=41)according to their native places.Differences in general clinical data,islet function,diabetic complications,and comorbidities were compared among the three groups.Results The age was higher in Hainan group than in South group(P<0.05).The HOMA-βwas lower in Hainan group than in North group(P<0.05).BMI,prevalence of DM family history,the proportion of education level above middle school and serum creatinine were lower,while HbA1 c,TC,LDL-C and the prevalence of diabetic peripheral neuropathy(DPN)were higher in Hainan group than in South group and North group(P<0.05 or P<0.01).Multiple linear regression analysis showed that education level,DM family history and LDL-C were the influencing factors for HbA1 c in Hainan group.Conclusion The newly diagnosed Han patients with T2 DM in Hainan had low-educational level,low prevalence of family history,low BMI,high blood glucose level,poor islet function,and high incidence of DPN.
