初次单侧人工全髋关节置换术围手术期异体红细胞输注相关因素分析

摘要 目的分析初次单侧人工全髋关节置换术围手术期异体红细胞输注相关因素。方法回顾性分析2016年3月至2020年1月在四川省南充市中医医院行单侧人工全髋关节置换术患者238例。收集可能影响患者围手术期异体红细胞输注的人口学资料和临床数据。分为输注组与未输注组,行组间比较及多因素分析。结果238例患者中117例(49.16%)接受异体红细胞输注,平均红细胞用量(4.51±1.03)U。多因素分析显示,术前血红蛋白水平在正常范围、自体血回输为初次单侧全髋关节置换术围手术期异体红细胞输注的保护性因素,年龄增长、手术时间延长、术后引流为危险因素(P均<0.05)。结论初次单侧人工全髋关节置换术围手术期异体红细胞输注率较高,主要受患者年龄、术前血红蛋白水平、手术时间、术后引流情况及自体血回输的影响,临床上可根据上述因素进行干预。 Objective To analyze the factors related to allogeneic red blood cell transfusion during the first unilateral total hip arthroplasty.Methods A retrospective analysis of 238 patients who underwent unilateral total hip arthroplasty in Nanchong Hospital of Traditional Chinese Medicine of Sichuan from March 2016 to January 2020 was performed.Demographic and clinical data that may affect patients with allogeneic red blood cell transfusion during the perioperative period was collected.According to whether the patients were transfused with allogeneic red blood cells,they were divided into infusion group and non-infusion group,and comparison between groups and multivariate analysis were performed.Results 117(49.16%)of 238 patients received allogeneic red blood cell infusion,the average dosage of red blood cell was(4.51±1.03)U.Multivariate analysis showed that preoperative hemoglobin level in the normal range,autologous blood transfusion were protective factors of allogeneic red blood cell transfusion during the perioperative period of primary unilateral total hip arthroplasty,age,prolonged operation time and postoperative drainage were risk factors(P all<0.05).Conclusion The rate of allogeneic red blood cell infusion during the first unilateral total hip arthroplasty is relatively high,mainly affected by the patient's age,hemoglobin level,operation time,postoperative drainage and autologous blood transfusion.Clinical intervention can be based on the above factors.
