8-OHDG、NSE及红细胞参数与HIE患儿脑损伤关系研究 被引量:1

RELATIONSHIP BETWEEN 8-OHDG NSE AND RED BLOOD CELL PARAMETERS AND BRAIN DAMAGE IN CHILDREN WITH HIE

摘要 目的探讨8-羟基脱氧鸟苷(8-OHDG)、神经元特异性烯醇化酶(NSE)、红细胞参数变化与缺氧缺血性脑病(HIE)患儿脑损伤的关系。方法随机选取2016年1月—2019年4月海南医学院第一附属医院儿科确诊的HIE患儿79例作为(HIE组)、同期健康新生儿80例作为对照组;检测对比二组的出生后3天的血清8-OHDG、NSE及红细胞参数水平,采用新生儿颅脑超声检测颅脑中央动脉平均血流速度(vs)、阻力指数(RI);并分析上述指标与脑灌注参数的关系。结果 HIE组患儿的8-OHDG、NSE、HCT、MCV、RWDCV、Ret%测定值均高于对照组,差异均有统计学意义(均P<0.05);HIE组患儿RBC测定值与对照组比较,差异无统计学意义(P>0.05);HIE组患儿的vs测定值低于对照组(P<0.05),HIE组患儿RI值高于对照组;重度组、中度组HIE患儿的8-OHDG、NSE、HCT、MCV、RWD-CV、Ret%测定值均高于轻度组;重度组HIE患儿的8-OHDG、NSE、HCT、MCV、RWD-CV、Ret%测定值均高于中度组;HIE患儿的8-OHDG、NSE、HCT、MCV、RWD-CV、Ret%测定值与vs呈负相关性;HIE患儿的8-OHDG、NSE、HCT、MCV、RWD-CV、Ret%测定值与RI呈正相关,以上差异均有统计学意义(均P<0.05)。结论 HIE患儿出现8-OHDG、NSE、HCT、MCV、RWD-CV、Ret%测定值升高,并且与病情程度有关,同时可在一定程度上反应患儿脑血流灌注情况。 Objective To investigate the relationship between 8-hydroxydeoxyguanosine(8-OHDG),neuron-specific enolase(NSE),red blood cell parameters and brain damage in children with hypoxic ischemic encephalopathy(HIE).Methods Seventy-nine children with HIE diagnosed in our hospital were selected as(HIE group) and 80 healthy newborns as control group.Serum 8-OHDG,NSE and erythrocyte parameters were compared between the two groups.The mean blood flow velocity(vs) and resistance index(RI) of the central cranial artery were detected by ultrasound.The relationship between the above indicators and cerebral perfusion parameters was analyzed.Results The values of 8-OHDG,NSE,HCT,MCV,RWD-CV and Ret% in children with HIE were higher than those in the control group, the difference was statistically significant(P<0.05).The RBC value of children in HIE group was compared with the control group.The difference was not statistically significant(P>0.05).The Vs of children in HIE group was lower than that of the control group(P<0.05).The RI value of children in HIE group was higher than that of the control group.Severe group and moderate degree The values of 8-OHDG,NSE,HCT,MCV,RWD-CV and Ret% in children with HIE were higher than those in mild group.8-OHDG,NSE,HCT and MCV in children with severe HIE The values of RWD-CV and Ret% were higher than those of the moderate group.The values of 8-OHDG,NSE,HCT,MCV,RWD-CV and Ret% in children with HIE were significantly negatively correlated with vs.Sexuality;the 8-OHDG,NSE,HCT,MCV,RWD-CV,and Ret% values of children with HIE were positively correlated with RI, the difference was all statistically significant(P<0.05).Conclusion The expression of 8-OHDG,NSE,HCT,MCV,RWD-CV and Ret% increased in children with HIE,and it was related to the degree of disease, and it could reflect the cerebral blood perfusion of children to some extent.
