摘要
目的分析钠-葡萄糖协同转运蛋白2(SGLT-2)抑制剂在糖尿病肾病(DN)中的应用效果。方法70例糖尿病肾病患者,依据随机数字表法分为常规组及SGLT-2抑制剂组,每组35例。常规组患者给予常规药物治疗,SGLT-2抑制剂组在常规组基础上增加SGLT-2抑制剂达格列净治疗。比较两组尿液泡沫消失时间、肢体肿胀消失时间、住院时间,治疗前后患者肾功能相关指标(24 h蛋白尿定量、血清白蛋白、血清尿素、肌酐)水平,治疗效果。结果SGLT-2抑制剂组尿液泡沫消失时间(6.42±1.34)d、肢体肿胀消失时间(6.21±1.03)d、住院时间(10.41±2.51)d短于常规组的(9.68±2.21)、(9.25±2.96)、(13.21±3.55)d,差异有统计学意义(P<0.05)。治疗后,两组患者24 h蛋白尿定量、血清白蛋白、血清尿素、肌酐均较治疗前降低,且SGLT-2抑制剂组患者24 h蛋白尿定量、血清白蛋白、血清尿素、肌酐低于常规组,差异有统计学意义(P<0.05)。SGLT-2抑制剂组治疗总有效率94.29%高于常规组的74.29%,差异有统计学意义(P<0.05)。结论常规药物联合SGLT-2抑制剂达格列净对于糖尿病肾病的治疗效果确切,可有效改善患者的症状和肾功能,缩短治疗时间,值得推广和应用。
Objective To analyze the practical effect of sodium-glucose cotransporter-2(SGLT-2)inhibitor on diabetic nephropathy.Methods A total of 70 patients with diabetic nephropathy were divided into conventional group and SGLT-2 inhibitor group according to random numerical table,with 35 cases in each group.The conventional group was treated with conventional medicine,and the SGLT-2 inhibitor group was treated with SGLT-2 inhibitor dapagliflozin on the basis of conventional group.The disappearance time of urine foam,disappearance time of limb swelling,hospitalization time,the level of renal function related indexes(24-urine protein excretion,serum albumin,serum urea,and creatinine)before and after treatment,and the therapeutic effect were compared between the two groups.Results The disappearance time of urine foam,disappearance time of limb swelling,hospitalization time of the SGLT-2 inhibitor group were(6.42±1.34),(6.21±1.03)and(10.41±2.51)d,which were shorter than(9.68±2.21),(9.25±2.96)and(13.21±3.55)d of the conventional group,and the difference was statistically significant(P<0.05).After treatment,the 24-urine protein excretion,serum albumin,serum urea and creatinine of the two groups were lower than those before treatment,and the SGLT-2 inhibitor group was lower than the conventional group.All the difference was statistically significant(P<0.05).The total effective rate of the SGLT-2 inhibitor group was 94.29%,which was higher than 74.29%of the conventional group,and the difference was statistically significant(P<0.05).Conclusion Combination of conventional medication and SGLT-2 inhibitor dapagliflozin has definite therapeutic effect on diabetic nephropathy,and it can effectively improve the symptoms and renal function of patients,shorten the treatment time,which is worthy of promotion and application.
