摘要
目的对Sysmex XN9000血细胞分析仪血小板聚集报警的可靠性进行评价。方法选取推片复查血小板的血标本1 075份,记录其在该分析仪检测时血小板聚集报警对应的Q-Flag值,以人工镜检为金标准,统计血小板聚集报警的真、假阴/阳性率,绘制ROC曲线分析该报警的最佳Q-Flag阈值,统计假阴性标本的血小板异常直方图报警率以判断该报警可否弥补血小板聚集报警的不足。结果血小板聚集报警的真阳性率、假阳性率分别为42.2%和84.8%,真阴性率、假阴性率分别为15.2%和57.8%;ROC曲线显示血小板聚集报警的最佳Q-Flag判断阈值为97;假阴性标本的血小板异常直方图报警率为60.6%。结论 Sysmex XN9000血小板聚集联合血小板异常直方图报警应用于血小板聚集与否的判断的假阴性率太高,血小板聚集报警的Q-Flag阈值临床应用价值不大。血小板复查需结合血常规复检规则中血小板的相关规则进行综合判断,必要时推片人工镜检。
Objective To evaluate the reliability of platelet aggregation alarm by Sysmex XN9000 hematology analyzer.Methods A total of 1 075 specimens were selected for platelet reexamination by blood smear. The result of manual microscope examination was used as a golden standard to judge the true or false positive/negative of platelet aggregation alarm by Sysmex XN9000 hematology analyzer,and the optimal Q-Flag threshold value of the alarm was analyzed based on ROC curve. At the same time,the platelet abnormal histogram alarm rate of false negative specimens was calculated to determine whether the platelet abnormal histogram alarm can make up for the deficiency of platelet aggregation alarm. Results The true or false positive/negative rates of platelet aggregation alarm were 42. 2%,84. 8%,15. 2% and 57. 8%. The ROC curve showed that the optimal cut-off for platelet aggregation alarm was 97. The platelet abnormal histogram alarm rate in false negative specimens was60. 6%. Conclusion The false negative rate of platelet aggregation combined with platelet abnormal histogram alarm by Sysmex XN9000 applied to the judgment of platelet aggregation or not is too high and the Q-Flag threshold of platelet aggregation alarm has little clinical significance. Platelet reexamination requires comprehensive judgment based on the relevant rules of platelet in the reexamination rules of the blood routine examination,if necessary,manual microscopy of the blood smear is required.
