摘要
目的探讨重组人组织纤溶酶原激活剂(rt-PA)联合高剂量瑞舒伐他汀治疗老年急性脑梗死合并高血脂的临床效果。方法选取平顶山市中医医院2017年5月至2019年5月收治的老年急性脑梗死合并高血脂患者133例,按随机数字表法分为2组:对照组66例予以rt-PA联合低剂量瑞舒伐他汀治疗,观察组67例予以rt-PA联合高剂量瑞舒伐他汀治疗。比较2组临床疗效、炎性因子[高敏C反应蛋白(hs-CRP)、白细胞介素(IL)-6、IL-10、IL-1β及肿瘤坏死因子-α(TNF-α)]水平、血脂[三酰甘油(TG)、总胆固醇(TC)及低密度脂蛋白(LDL-C)]水平及不良反应发生情况。结果与对照组比较,观察组治疗总有效率显著升高,差异有统计学意义(P<0.05)。与治疗前比较,2组治疗后hs-CRP、IL-6、TNF-α、IL-1β、LDL-C、TG及TC水平均显著降低,且观察组降低更为明显,差异均有统计学意义(均P<0.05);2组治疗后IL-10水平显著升高,且观察组升高更为明显,差异均有统计学意义(均P<0.05)。与对照组比较,观察组不良反应发生率差异无统计学意义(χ^(2)=0.802,P>0.05)。结论rt-PA联合高剂量瑞舒伐他汀可改善老年急性脑梗死合并高血脂患者炎症反应,降低血脂水平,提高治疗效果且不良反应少。
Objective To investigate the clinical effect of recombinant human tissue plasminogen activator(rt-PA)combined with high-dose rosuvastatin on senile acute cerebral infarction complicated by hyperlipidemia.Methods A total of 133 elderly patients with acute cerebral infarction complicated by hyperlipidemia who were admitted to Pingdingshan Hospital of Traditional Chinese Medicine from May 2017 to May 2019 were randomly divided into two group.The control group(n=66)was treated with rt-PA and low-dose rosuvastatin,and the observation group(n=67)was given rt-PA and high-dose rosuvastatin.Clinical efficacy,adverse reactions and levels of high sensitivity C-reactive protein(hs-CRP),interleukin-6(IL-6),interleukin-10(IL-10),interleukin-1β(IL-1β),tumor necrosis factor-α(TNF-α),triglyceride(TG),total cholesterol(TC)and low-density lipoprotein cholesterol(LDL-C)were compared between the two groups.Results The total effective rate in the observation group was higher than that in the control group(P<0.05).After treatment,the levels of hs-CRP,IL-6,TNF-α,IL-1β,LDL-C,TG and TC decreased but the concentrations of IL-10 increased in both groups,and the changes in the observation group were more obvious than those in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(χ^(2)=0.802,P>0.05).Conclusion The combined treatment with rt-PA and high-dose rosuvastatin can improve therapeutic effect and reduce inflammatory response,blood lipid levels and adverse reactions in elderly patients with acute cerebral infarction complicated by hyperlipidemia.
