广州市2009—2020年新报告15~24岁HIV/AIDS人群基本特征及趋势分析 被引量:7

摘要 目的:分析2009—2020年广州市新报告15~24岁HIV感染人群特征及变化趋势,为科学制定15~24岁人群艾滋病防治措施提供依据。方法:资料来源于国家艾滋病防治基本信息系统2009—2020年新报告HIV/AIDS数据库的15~24岁人群,采用Joinpoint回归模型进行分析。结果:新报告15~24岁HIV/AIDS病例共3 375例,男女性别比10.56∶1,诊断时平均年龄(21.56±2.05)岁;性传播为主,占97.75%(3 299/3 375),其中男男同性传播占70.64%(2 384/3 375);资料来源为检测咨询占42.96%(1 450/3 375);学生占18.19%(614/3 375);15~24岁人群晚发现比例13.48%(455/3 375),较全年龄组晚发现占比少(χ^(2)=224.90,P<0.05)。2009—2020年15~19岁(AAPC=17.84)、15~24岁(AAPC=14.55)、20~24岁(AAPC=13.94)年龄组的新报告病例数均呈上升趋势,且增长趋势均比全年龄组快(AAPC=10.02);15~19岁增长趋势最快。15~24岁男性新报告病例数增长趋势快于女性(AAPC=17.53)。经同性传播途径病例数呈增长趋势(AAPC=22.42)。学生病例报告数呈增长趋势,学生(AAPC=23.65)增长趋势比非学生(AAPC=12.82)快。结论:广州市15~24岁HIV/AIDS新报告病例数的增长趋势上升较快,低年龄组和校内外青年流行形势均不容乐观。应重视15~24岁人群的性教育和艾滋病防治工作,采取积极有效的干预措施。 Objective:To analyze the characteristics and trends of newly reported HIV/AIDS among people aged 15~24 in Guangzhou, 2009—2020 and provide evidence for AIDS prevention and control. Methods: Datas were collected from the Basic Web-based HIV/AIDS Case Reporting System. The HIV infection cases in young people aged 15~24 between 2009 and 2020 were selected. The basic characteristics were described, and Joinpoint Regression Model was used to analyze the temporal trends of new diagnosis rates in different age groups. Results: A total of 3 375 HIV/AIDS cases were reported in young people, the male to female ratio was 10.56∶1, and the mean diagnosis age was 21.56±2.05 years. Sexually transmitted were 97.75%(3 299/3 375), of which 70.64%(2 384/3 375) were homosexual transmission;tested and consulted were 42.96%(1450/3 375);students were 18.19%(614/3 375).The rate of late detection was 13.48%(455/3 375) in the 15~24 age group less than that in the whole age group( χ2=224.90,P<0.05). In 2009—2020, the number of newly reported cases among people aged 15~19(AAPC=17.84),15~24(AAPC=14.55) and 20~24(AAPC=13.94) all showed a rising trend, three age group growing faster than the adult group(AAPC=10.02),15~19 years old increased the most. The number of newly reported cases among young men aged 15~24 was higher than among young women(AAPC=17.53). The number of cases transmitted by homosexual transmission increased(AAPC=22.42). The number of cases reported by students(AAPC=23.65) showed an increasing trend, which was faster than that of non-students(AAPC=12.82). Conclusions: The number of newly reported HIV/AIDS cases aged 15~24 in Guangzhou were rising rapidly, the epidemic situation of the younger age group and out-of-school group were not optimistic. Attention should be paid to sex education and AIDS prevention and control among people aged 15~24, and active and effective intervention measures should be taken.
