摘要
目的:探讨结石封堵器在输尿管镜治疗输尿管上段结石中的应用效果。方法:回顾性分析2019年1月-2020年12月本院收治的60例输尿管上段结石患者的临床资料。按照术中是否使用结石封堵器分为两组,将使用结石封堵器的28例患者设为观察组,未使用结石封堵器的32例患者设为对照组。对照组行输尿管镜下碎石术治疗,观察组使用结石封堵器联合输尿管镜下碎石术治疗。比较两组围术期相关指标、碎石成功情况、并发症发生情况。结果:观察组手术时间、肾积水消退时间、血尿清除时间、住院时间均短于对照组,术中出血量低于对照组,差异均有统计学意义(P<0.05)。观察组碎石成功率为96.43%,高于对照组的71.88%,差异有统计学意义(P<0.05)。观察组并发症发生率为3.57%,低于对照组的28.13%,差异有统计学意义(P<0.05)。结论:输尿管镜治疗输尿管上段结石中使用结石封堵器能够减小手术创伤,提高碎石成功率,缩短肾积水消退时间和血尿清除时间,减少并发症发生,加快术后恢复进程。
Objective:To investigate the application effect of calculi occluder in the treatment of upper ureteral calculi by ureteroscopy.Method:The clinical data of 60 patients with upper ureteral calculi admitted to our hospital from January 2019 to December 2020 were retrospectively analyzed.According to whether calculi occluders were used during the operation,28 patients using calculi occluders were set as the observation group,and 32 patients without calculi occluders were set as the control group.The control group was treated with ureteroscopic lithotripsy,while the observation group was treated with calculi occluder combined with ureteroscopic lithotripsy.The perioperative related indexes,the success of lithotripsy and the incidence of complications were compared between the two groups.Result:The operation time,hydronephrosis regression time,hematuria clearance time and hospitalization time of the observation group were shorter than those of the control group,the intraoperative blood loss was lower than that of the control group,the differences were statistically significant (P<0.05).The success rate of lithotripsy in the observation group was 96.43%,which was higher than 71.88% in the control group,the difference was statistically significant (P<0.05).The complication rate of the observation group was 3.57%,which was lower than 28.13% of the control group,the difference was statistically significant (P<0.05).Conclusion:Ureteroscopic treatment of upper ureteral calculi using calculi occluder can reduce the surgical trauma,improve the success rate of lithotripsy,shorten the hydronephrosis regression time and hematuria clearance time,reduce the occurrence of complications,accelerate the process of postoperative recovery.
