摘要
目的糖尿病足动脉血管早期病变临床诊断中彩色多普勒超声的应用分析。方法将该院在2018年10月—2020年10月间收诊的60例疑似糖尿病足动脉血管早期病变患者纳为研究,分别开展超声造影检查与彩色多普勒超声检查,以X线数字减影血管造影检查(DSA)结果为金标准,对比分析上述两种检查结果的阳性预测值、阴性预测值、灵敏度、特异性以及血管病变检出情况。结果60例疑似糖尿病足动脉血管早期病变患者经DSA检查结果显示,有53例为糖尿病足动脉血管早期病变;经超声造影检查结果显示,有47例为阳性,13例为阴性;阳性预测值为89.36%、阴性预测值为15.38%、灵敏度为79.25%、特异性为28.57%;而经彩色多普勒超声检查结果显示,有51例为阳性,9例为阴性;阳性预测值为94.12%、阴性预测值为44.44%、灵敏度为90.57%、特异性为57.14%;相应数据比较,差异无统计学意义(P>0.05)。另对DSA确诊的53例患者进行血管病变检查,DSA、超声造影、彩色多普勒超声的结果基本等同,差异无统计学意义(P>0.05)。结论彩色多普勒超声检查糖尿病足动脉血管早期病变有较好的灵敏度与特异性,漏诊误诊风险低,有利于患者及时接受适宜治疗,具有临床推广应用价值。
Objective To analyze the application of color Doppler ultrasound in the clinical diagnosis of early diabetic foot arterial disease.Methods 60 patients with suspected early disease of the arteries of the diabetic foot arteries in the hospital between October 2018 and October 2020 were included as study cases.Digital subtraction angiography(DSA)results were the gold standard,and the positive predictive value,negative predictive value,sensitivity,specificity,and detection of vascular lesions of the two test results were compared and analyzed.Results 60 cases of suspected diabetic foot arterial vascular disease showed that 53 cases were early diabetic foot arterial vascular disease.The results of contrast-enhanced ultrasound showed that 47 cases were positive and 13 cases were negative;positive predictive value was 89.36%,the negative predictive value was 15.38%,the sensitivity was 79.25%,and the specificity was 28.57%.The results of color Doppler ultrasound showed that 51 cases were positive and 9 cases were negative;the positive predictive value was 94.12%,the negative predictive value was 44.44%,the sensitivity was 90.57%,and the specificity was 57.14%;the corresponding data comparison showed that there was no statistically significant statistical difference between the groups(P>0.05).In addition,53 patients diagnosed with DSA were examined for vascular lesions.The results of DSA,contrast-enhanced ultrasound,and color Doppler ultrasound were basically the same,and the difference was not statistically significant(P>0.05).Conclusion Color Doppler ultrasonography has good sensitivity and specificity in detecting early disease of diabetic foot arteries,and the risk of misdiagnosis is low.It is helpful for patients to receive appropriate treatment in time,and has clinical application value.
