高压氧联合有氧运动治疗老年轻度血管性痴呆的疗效观察 被引量:7

摘要 目的探究高压氧联合有氧运动治疗老年轻度血管性痴呆(mVD)的治疗效果及安全性。方法选取上海中冶医院金惠康复医院收治的老年mVD患者100例作为研究对象。根据随机数字表原则,将患者分为4组,每组各25例。对照组患者采用奥拉西坦治疗,有氧运动组在此基础上联合有氧运动,高压氧组在此基础上联合高压氧,联合组患者在此基础上联合有氧运动和高压氧治疗。比较各组患者治疗前、治疗8周后简易智力状态检查(MMSE)评分、蒙特利尔认知评估量表(MoCA)评分、血清磷酸化Tau(P-tau)、尿AD7C-NTP水平变化情况及治疗期间不良反应。结果经过治疗后,各组MMSE评分、MoCA评分均高于治疗前(P<0.001)。有氧运动组、高压氧组、联合组MMSE评分、MoCA评分均高于对照组(P<0.05)。联合组MMSE评分、MoCA评分高于有氧运动组、高压氧组(P<0.05)。治疗后,各组P-tau水平均低于治疗前(P<0.01)。有氧运动组、高压氧组、联合组P-tau水平低于对照组,其中,联合组P-tau水平最低(P<0.01)。经过治疗后,联合组AD7C-NTP水平低于治疗前,并且低于对照组(P<0.05)。各组间不良反应发生率比较,差异无统计学意义(P>0.05)。结论奥拉西坦、有氧运动和高压氧均可有效改善老年mVD患者认知功能和P-tau水平,但三者联合应用具有更确切的治疗效果,同时可降低患者尿AD7C-NTP水平,安全性较好。 Objective To analyze the therapeutic effect and safety of hyperbaric oxygen combined with aerobic exercise on mild vascular dementia(mVD).Methods 100 older patients with mVD admitted to Shanghai MCC Hospital were selected.According to random number table,the patients were divided into 4 groups with 25 cases in each group.The patients in the control group were treated with oxiracetam.The patients in the aerobic exercise group were treated with oxiracetam plus aerobic exercise.The patients in the hyperbaric oxygen group were treated with oxiracetam plus hyperbaric oxygen.The patients in the combined group were treated with oxiracetam plus aerobic exercise and hyperbaric oxygen.Mini-mental state examination(MMSE)score,montreal cognitive assessment scale(MoCA)score,serum phosphorylated tau(P-tau)level and urine AD7C-NTP level were evaluated before and after 8 weeks of treatment.Adverse reactions during the threapy were documented.Results After treatment,MMSE score and MoCA score in each group were higher than those before treatment(P<0.001).The MMSE score and MoCA score in the aerobic exercise group,hyperbaric oxygen group and combined group were higher than those in the control group(P<0.05).The MMSE score and MoCA score in the combined group were higher than those in the aerobic exercise group and the hyperbaric oxygen group(P<0.05).After treatment,P-tau levels in each group were lower than those before treatment(P<0.01).The levels of P-tau in the aerobic exercise group,hyperbaric oxygen group and combined group were lower than those in the control group,and the level in the combined group was lowest(P<0.01).After treatment,the level of AD7C-NTP in the combined group was lower than that before treatment and lower than that in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions among the groups(P>0.05).Conclusion Oxiracetam,aerobic exercise and hyperbaric oxygen may improve the cognitive function and P-tau level respectively,but the combination of the three methods may be more effective and safe,and meanwhile reduce the level of urine AD7C-NTP in patients with mVD.
