载脂蛋白E及MASP-2基因多态性与颅脑损伤术后肺部感染易感性关系 被引量:1

Relationship of apolipoprotein E and MASP-2 gene polymorphisms with susceptibility to postoperative pulmonary infection in patients with brain injury

摘要 目的探讨载脂蛋白E(APOE)及甘露糖结合凝集素相关丝氨酸蛋白酶2(MASP-2)基因多态性与颅脑损伤患者术后肺部感染易感性的关系。方法收集2018年12月-2019年12月武汉市普仁医院诊治的126例颅脑损伤患者临床资料,依据患者术后是否发生肺部感染分为感染组和未感染组。比较两组临床资料,检测两组样本APOE和MASP-2的基因多态性,分析APOE和MASP-2的基因多态性与颅脑损伤患者术后肺部感染易感性的相关性。结果126例颅脑损伤患者术后43例发生肺部感染,占34.13%;感染组与未感染组入院时GCS评分、术后卧床时间、术前血清白蛋白水平比较差异有统计学意义(P<0.05)。感染组APOE等位基因ε3分布频率低于未感染组,而等位基因ε4的分布频率则高于未感染组(P<0.05)。感染组MASP-2基因位点rs72550870基因型AA分布频率和等位基因A分布频率高于未感染组(P<0.05)。多因素非条件Logistic回归分析提示APOE等位基因ε4和MASP-2位点rs72550870基因型AA和等位基因A与颅脑损伤患者术后肺部感染的发生有关(P<0.001)。结论APOE和MASP-2的基因多态性与颅脑损伤患者术后肺部感染易感性密切相关,其中APOE等位基因ε4和MASP-2位点rs72550870基因型AA和等位基因A的存在是颅脑损伤患者术后肺部感染风险增高的影响因素。 OBJECTIVE To investigate the relationship between apolipoprotein E(APOE)and mannose-binding lectin-associated serine protease-2(MASP-2)gene polymorphisms and susceptibility to postoperative pulmonary infection in patients with brain injury.METHODS The clinical data of 126 patients with brain injury diagnosed and treated in Wuhan Puren Hospital from Dec 2018 to Dec 2019 were collected.According to the presence or absence of postoperative infection,the patients were divided into the infected group and uninfected group.Clinical data of the two groups were compared.APOE and MASP-2 gene polymorphisms were detected,and their correlation with susceptibility to postoperative pulmonary infection in patients with brain injury was analyzed.RESULTS In this study,pulmonary infection occurred in 43 of 126 cases after operation,accounting for 34.13%.There were significant differences in GCS score at admission,postoperative bed time and preoperative serum albumin level between the infected group and the uninfected group(P<0.05).The APOE alleleε3 of the infection group,was lower than that in the uninfected group,while the alleleε4 of the infection group,was higher than that in the uninfected group(P<0.05).The frequencies of genotype AA and allele A of MASP-2 gene(rs72550870)in the infected group were significantly higher than those in the uninfected group(P<0.05).Multivariate unconditional Logistic regression analysis suggested that APOE allele albumin level was significantly lower in the infected grassociated with the occurrence of postoperative pulmonary infection in patients with brain injury(P<0.001).CONCLUSION APOE and MASP-2 gene polymorphisms are closely related to susceptibility to postoperative pulmonary infection in patients with brain injury.APOE allele,MASP-2,rs72550870 site and the allele A were high risk factors for pulmonary infection in patients with brain injury.
