摘要
目的:观察多奈哌齐联合氯氮平对阿尔茨海默病(AD)患者精神障碍及生活能力的影响。方法:AD患者108例随机为对照组和观察组各54例。对照组患者给予盐酸多奈哌齐,观察组在对照组基础上加用氯氮平治疗。两组均连续给药6个月。观察两组临床疗效和药品不良反应发生情况,比较两组患者治疗前后认知功能(MMSE)、行为和精神障碍(BEHAVE-AD、BPRS)、日常生活能力(ADL)、氧化应激指标(SOD、皮质醇)等的变化情况。结果:观察组临床总有效率为90.74%,明显高于对照组的74.07%(P<0.05)。治疗后,两组患者的MMSE评分和ADL评分均较前明显升高,BEHAVE-AD、BPRS评分则较前明显降低(P<0.05);且观察组各项量表评分均明显优于对照组(P<0.05)。治疗后两组患者血清SOD、皮质醇水平均较前明显升高(P<0.05),且观察组血清SOD水平高于对照组(P<0.05),皮质醇水平低于对照组(P<0.05)。两组药品不良反应发生率差异无统计学意义(P>0.05)。结论:多奈哌齐联合氯氮平治疗AD疗效确切,可有效改善患者的认知能力和行为与精神障碍,提高日常生活活动能力,改善氧化应激水平,且安全性较高。
Objective:To observe the effects of donepezil combined with clozapine on mental disorder and life ability of patients with Alzheimer’s disease(AD).Methods:Totally 108 patients with AD were randomly divided into control group and observation group with 54 cases in each group.The control group was given donepezil hydrochloride,and the observation group was treated with clozapine on the basis of the control group.Both groups were treated for 6 months.The clinical efficacy and adverse drug reactions of the two groups were observed,and the changes of cognitive function(MMSE),behavioral and mental disorders(BEHAVE-AD,BPRS),activities of daily living(ADL)and oxidative stress index(SOD,cortisol)were compared before and after treatment.Results:The total effective rate of the observation group was 90.74%,which was significantly higher than that(74.07%)of the control group(P<0.05).After treatment,MMSE score and ADL score of the two groups were significantly higher than those before treatment,while BEHAVE-AD and BPRS scores were significantly lower than those before treatment(P<0.05).After treatment,the levels of SOD and cortisol in the two groups were significantly higher than those before treatment(P<0.05),and the level of SOD in the observation group was higher than that in the control group(P<0.05),and the level of cortisol in the observation group was lower than that in the control group(P<0.05).There was no significant difference in the incidence of adverse drug reactions between the two groups(P>0.05).Conclusion:Donepezil combined with clozapine is effective in the treatment of AD,which can effectively improve the cognitive ability,behavioral and mental disorders,activities of daily living and level of oxidative stress with high safety.
