摘要
目的分析乙酰半胱氨酸泡腾片联合布地奈德雾化吸入治疗脑梗死并发肺部感染患者的效果。方法选取2016年7月—2019年7月本院收治的脑梗死并发肺部感染102例,根据治疗方案的不同均分为研究组(给予乙酰半胱氨酸泡腾片联合布地奈德雾化吸入治疗)和对照组(仅给予布地奈德雾化吸入治疗),均持续治疗7 d。比较2组临床效果、症状体征改善情况及住院时间,2组治疗前后神经功能、肺部感染改善情况和血气指标、肺功能指标水平,以及2组治疗后不良反应发生情况。结果研究组总有效率高于对照组(P<0.01)。研究组呼吸困难缓解时间、止咳时间、退热时间、肺部啰音消失时间及住院时间均短于对照组(P<0.01)。与治疗前比较,2组美国国立卫生研究院卒中量表评分、临床肺部感染评分及二氧化碳分压水平均降低,氧分压、血氧饱和度、第一秒用力呼气容积占预计值的百分比水平均明显升高,其中以研究组改善最为显著,差异有统计学意义(P<0.05,P<0.01)。2组治疗后不良反应总发生率比较差异无统计学意义(P>0.05)。结论乙酰半胱氨酸泡腾片联合布地奈德雾化吸入治疗脑梗死并发肺部感染效果显著,可缓解患者临床症状,改善肺功能及血气指标,促进脑神经功能恢复,且安全性高。
Objective To analyze clinical efficacy of Acetylcysteine Effervescent Tablets combined with Budesonide Atomization Inhalation in treatment of patients with cerebral infarction complicated by pulmonary infection.Methods A total of 102 patients with cerebral infarction complicated by pulmonary infection admitted between July 2016 and July 2019 were divided into research group(treated with Acetylcysteine Effervescent Tablets combined with Budesonide Atomization Inhalation)and control group(treated with Budesonide Atomization Inhalation only)according to different therapeutic methods.Both groups were continuously treated for 7 d.Clinical efficacy,improvement conditions of symptoms and signs and length of hospital stay were compared,and improvement conditions of neurological function and pulmonary infection,levels of blood gas index and pulmonary function index before and after treatment,as well as incidence rate of adverse reactions after treatment were also compared between two groups.Results The total effective rate in research group was significantly higher than that in control group(P<0.01).Times of dyspnea relief,cough relief,pyretolysis,disappearance of pulmonary rales and duration of hospital stay in research group were significantly shorter than those in control group(P<0.01).Compared with those before treatment,National Institutes of Health Stroke Scale(NIHSS)scores,clinical pulmonary infection scores and levels of carbon dioxide partial pressure were significantly decreased,while values of oxygen partial pressure,blood oxygen saturation and percentages of forced expiratory volume in one second to predicted value were significantly increased in two groups,and the improved conditions were more significant in research group(P<0.05,P<0.01).There was no significant difference in the total incidence rate of adverse reactions after treatment between two groups(P>0.05).Conclusion Acetylcysteine Effervescent Tablets combined with Budesonide Atomization Inhalation is effective in treatment of patients with cerebral infarction complicated by pulmonary infection.It may alleviate clinical symptoms,improve pulmonary function and blood gas indexes and promote the recovery of cranial nerves function with good safety.
