急性有机磷农药中毒患者血糖、白细胞、CRP和乙酰胆碱酯酶的变化及其临床意义 被引量:3

摘要 目的观察乙酰胆碱酯酶、血糖、CRP、白细胞在急性有机磷农药中毒者中的变化情况。方法选择2019年7月-2020年7月急性有机磷农药中毒者93例,健康者93名为课题观察样本,分别归纳为中毒组、健康组。后依据中毒程度对中毒组行细致划分,明确各组别中相关指标的变化情况。结果在各指标数据中,中毒组乙酰胆碱酯酶数值是(2769.54±982.88)U/L,CRP数值是(34.72±5.02)mg/L,血糖数值是(7.87±1.69)mmol/L,白细胞数值为(15.05±5.22)×10^(9)/L,与健康组相比,差异有统计学意义(t=31.718、29.741、20.478、16.414,P<0.05)。以中毒程度划分为重、中、轻度组后,3组乙酰胆碱酯酶、CRP、血糖、白细胞数据差异有统计学意义(P<0.05),其中CRP、血糖、白细胞和中毒程度的关系为正相关,而乙酰胆碱酯酶与此的关系为负相关。3组中毒指数、住院时间、APACHE-Ⅱ数据差异有统计学意义(P<0.05),各指标和中毒程度间的关系为正相关。结论乙酰胆碱酯酶、白细胞、血糖、CRP不仅能对急性有机磷农药中毒者行鉴别诊断,且有较高的中毒严重程度区分价值,对中毒者预后治疗有参考作用。 Objective To observe the changes of acetylcholinesterase,blood glucose,CRP and white blood cells in patients with acute organophosphorus pesticide poisoning.Methods From July 2019 to July 2020,93 cases of acute organophosphorus pesticide poisoning and 93 healthy subjects were selected as subject observation samples,which were respectively classified into poisoning group and healthy group.Then,the poisoning group was divided according to the degree of poisoning,and the changes of related indicators in each group were clarified.Results In each index data,the value of acetylcholinesterase in the poisoned group was(2769.54±982.88)U/L,the value of CRP was(34.72±5.02)mg/L,the value of blood glucose was(7.87±1.69)mmol/L,and the value of white blood cells was(15.05±5.22)×10^(9)/L,compared with the healtly group,the difference was statistically significant(t=31.718,29.741,20.478,16.414,P<0.05).After being divided into severe,moderate,and mild groups according to the degree of poisoning,the differences in the data of acetylcholinesterase,CRP,blood glucose,and white blood cells in the three groups were statistically significant(P<0.05),and the relationship between CRP,blood glucose,white blood cells and the degree of poisoning was positive correlation,and the relationship between acetylcholinesterase and this was a negative correlation.The differences in the three groups of poisoning index,length of stay,and APACHE-Ⅱdata were significantly different(P<0.05),and the relationship between each index and the degree of poisoning was positively correlated.Conclusion Acetylcholinesterase,leukocytes,blood glucose,and CRP can not only differentiate the patients with acute organophosphorus pesticide poisoning,but also have a higher value for distinguishing the severity of poisoning,which can be used as a reference for the prognostic treatment of poisoned patients.
