摘要
目的分析系统护理宣教对于妊娠期糖尿病(GDM)的护理效果。方法患者诊治时间介于2018年8月—2020年7月间,共计GDM患者48例。以抽签法为准,A组计入例数25,予以系统护理宣教;B组计入例数23,予以常规健康教育。对比护理效果。结果护理前,两组的空腹血糖(FPG)、餐后2 h血糖(2 hPG)、糖化血红蛋白(HbA1c)等血糖指标和焦虑、抑郁评分对比,差异无统计学意义(P>0.05)。护理后,A组血糖指标数值均低于B组,焦虑抑郁评分低于B组,差异有统计学意义(P<0.05)。A组无新生儿窒息情况,出现1例(4.00%)巨大儿和2例(8.00%)剖宫产,不良妊娠结局总概率为12.00%;B组出现4例(17.39%)新生儿窒息,6例(26.09%)巨大儿,4例(17.39%)剖宫产,不良妊娠结局总概率为60.87%,两组总概率对比,差异有统计学意义(P<0.05)。护理后,A组的GDM自我效能评分明显高于B组,差异有统计学意义(P<0.05)。结论为GDM患者实行系统护理宣教能够降低血糖数值,缓解其焦虑与抑郁心理,且能显著提升产妇的自我效能,进而减少产后抑郁等情况,改善妊娠结局。
Objective To analyze the effect of systematic nursing education on gestational diabetes(GDM).Methods The diagnosis and treatment period of patients was between August 2018 and July 2020,with a total of 48 GDM patients.Based on the lottery method,group A was included in the number of cases and received systematic nursing education;group B was included in the number of cases and received regular health education.Compared the effect of care.Results Before nursing,there was no difference in the comparison of fasting plasma glucose(FPG),2 h postprandial plasma glucose(2 h PG),glycosylated hemoglobin(HbA1 c)and anxiety and depression scores between the two groups,the differences was not statistically significant(P>0.05).After nursing,the blood glucose index values of group A were lower than those of group B,and the anxiety and depression scores were lower than those of group B.The differences was statistically significant(P<0.05).There was no neonatal asphyxia in group A,with 1 case(4.00%)of macrosomia and 2 cases(8.00%)of cesarean section.The overall probability of adverse pregnancy outcome was 12.00%;group B had 4 cases(17.39%)of neonatal asphyxia.6 cases(26.09%)of giant infants,4 cases(17.39%)of cesarean section,the total probability of adverse pregnancy outcome was 60.87%,and the total probability of the two groups was compared,the differences was statistically significant(P<0.05).After nursing,the GDM self-efficacy score of group A was significantly higher than that of group B,the differences was statistically significant(P<0.05).Conclusion Implementing systematic nursing education for patients with GDM can reduce blood glucose levels,relieve their anxiety and depression,and can significantly improve the self-efficacy of the mothers,thereby reducing postpartum depression and other conditions,and improving pregnancy outcomes.
