摘要
目的观察老年2型糖尿病(T2DM)患者中肌肉减少症(肌少症)的发生情况,分析肌少症发生的影响因素。方法回顾分析150例老年T2DM患者的病历资料,统计患者肌少症的发生情况及主要的基线资料,比较发生与未发生肌少症患者的主要资料,经单因素与多因素分析找出可能导致肌少症发生的影响因素。结果150例老年T2DM患者中,有27例患者发生肌少症,发生率为18.00%;发生与未发生肌少症患者的神经血管系统并发症情况、性别、糖化血红蛋白(HbA1c)、铁蛋白(SF)、25羟基维生素D[25(OH)D]水平比较差异有统计学意义(P<0.05);逐步纳入相关资料进行Logistic回归分析结果显示,有神经血管系统并发症、男性、HbA1c过表达、SF过表达、25(OH)D低表达均是老年T2DM患者肌少症发生的影响因素(OR>1,P<0.05)。结论老年T2DM患者存在神经血管系统并发症、男性、HbA1c过表达、SF过表达、25(OH)D低表达等情况可能会增加肌少症发生风险,建议临床上重视存在上述风险因素的老年T2DM患者,进行早期合理干预。
Objective To observe the occurrence of sarcopenia in elderly patients with type 2 diabetes mellitus(T2DM),and analyze the influencing factors of sarcopenia.Methods A retrospective analysis of 150 elderly T2DM patients'medical records was performed.By statistical analysis of the occurrence and main baseline data of sarcopenia,and comparison of the main data of patients with and without sarcopenia,the influencing factors that might lead to sarcopenia were found out on the basis of univariate and multivariate analysis.Results Among 150 elderly patients with T2DM,27 patients had myopenia,and the incidence rate was 18.00%.There were significant differences in neurovascular complications,gender,glycosylated hemoglobin(HbA1c),ferritin(SF)and 25 hydroxyvitamin d[25(OH)D]between patients with and without myopenia(P<0.05).Relevant data were gradually included for Logistic regression analysis,and the results showed that having complications caused by neurovascular system,male,overexpression of HbA1c and SF as well as low-level of 25(OH)D were the influencing factors of myopenia in elderly T2DM patients(OR>1,P<0.05).Conclusions Elderly T2DM patients with complications caused by neurovascular system,male,overexpression of HbA1c and SF as well as low-level of 25(OH)D may increase the risk of sarcopenia.It is suggested that clinical medical staff should attach importance to early and reasonable intervention toward elderly T2DM patients with the above risk factors.
