摘要
目的:分析带蒂瓣联合结缔组织移植术改善牙龈瘤切除术后根面暴露的效果。方法:纳入牙龈瘤患者60例,按随机数表法分为3组(n=20),分别采取翻瓣术(A组)、带蒂瓣术(B组)、带蒂瓣联合结缔组织移植术(C组)。比较3组患者术后即刻、术后3、6个月的角化龈宽度、根面覆盖率、术后美学RES评分及患者满意度VAS评分。结果:术后3、6个月,C组的角化龈宽度明显高于A组、B组(P<0.05);A组的根面覆盖率和术后RES均明显低于B组、C组(P<0.05);C组的VAS均明显高于A组、B组(P<0.05)。结论:相较于翻瓣术,应用带蒂瓣术及带蒂瓣联合结缔组织移植术治疗能有效改善牙龈瘤切除术后的根面暴露,恢复角化龈宽度,增加患者满意度。
Objective:To observe the effects of pedicled flap combined with connective tissue transplantation for the improvement of root surface exposure after gingival tumor resection.Methods:60 patients with gingival tumors were divided into 3 groups(n=20)and treated by flap surgery(group A),pedicle flap surgery(group B)and pedicle flap combined with connective tissue transplantation(group C)respectively.The keratinized gingival width,root coverage,postoperative aesthetics RES and satisfaction VAS scores of the 3 groups immediately after surgery,3 and 6 months after surgery were compared.Results:At 3 and 6 months after surgery,the width of keratinized gingiva in group C was higher than that in groups A and B(P<0.05);the root coverage and postoperative RES of group A were lower than those in groups B and C(P<0.05);the VAS of group C was higher than that of group A and group B(P<0.05).Conclusion:Compared with flap surgery,pedicled flap combined with connective tissue transplantation can effectively improve the root surface exposure after gingival tumor resection can restore the width of keratinized gingival and increase patient satisfaction.
