摘要
目的观察静脉应用地塞米松对单节段颈椎前路椎间盘切除减压融合术(ACDF)术后吞咽困难及椎前软组织肿胀程度的影响。方法选取2018年9月至2020年12月本科室因颈椎病行择期ACDF的87例患者为研究对象,随机将其分为对照组(43例)与激素组(44例)。对照组术后常规给予抗生素及雾化吸入等对症支持治疗,激素组在此基础上静脉输入地塞米松。比较两组的治疗效果。结果术后3个月,激素组的吞咽困难发生率显著低于对照组(P<0.05)。术后1、3、5 d,激素组的VAS评分均显著低于对照组(P<0.05)。术后5 d、3个月,激素组的椎前软组织肿胀程度均显著低于对照组(P<0.05)。术后3 d,激素组的白细胞计数和C反应蛋白水平均显著低于对照组(P<0.05)。结论地塞米松用于单节段ACDF术后能够有效减轻颈椎椎前软组织肿胀程度、术区疼痛,降低吞咽困难发生率,减轻局部炎症水平。
Objective To observe the effects of intravenous dexamethasone on dysphagia and degree of prevertebral soft tissue swelling after single-segment anterior cervical decompression and fusion(ACDF).Methods A total of 87 patients who underwent elective ACDF due to cervical spondylosis in our department from September 2018 to December 2020 were selected as the study objects and randomly divided into control group(43 cases)and hormone group(44 cases).The control group was routinely given symptomatic supportive treatment such as antibiotics and atomization inhalation after operation,while the hormone group was given intravenous dexamethasone on this basis.The therapeutic effects of the two groups were compared.Results At 3 months after operation,the incidence of dysphagia in the hormone group was lower than that in the control group(P<0.05).At 1,3 and 5 d after operation,VAS scores in the hormone group were significantly lower than those in the control group(P<0.05).At 5 d and 3 months after operation,the degree of prevertebral soft tissue swelling in the hormone group were significantly lower than those in the control group(P<0.05).At 3 d after operation,the WBC and C-reactive protein level in the hormone group were significantly lower than those in the control group(P<0.05).Conclusion Dexamethasone can effectively reduce the degree of prevertebral soft tissue swelling of cenvical vertebra,pain in the operative area,the incidence of dysphagia and local inflammation levels after single-segment ACDF.
