高频超声与磁共振成像对踝关节外侧副韧带损伤的诊断价值 被引量:5

Diagnostic Value of High Frequency Ultrasound and Magnetic Resonance Imaging in Injury of Ankle Joint Lateral Collateral Ligament

摘要 目的:研究高频超声与磁共振成像(MRI)对踝关节外侧副韧带(LCL)损伤的诊断价值。方法:选择2019年5月-2021年5月于本院治疗的93例疑为踝关节LCL损伤的患者。患者均采用超声与MRI检查。以手术探查或手术修补结果作为金标准,计算高频超声与MRI诊断踝关节LCL损伤的敏感度、特异度及准确率,比较高频超声与MRI对踝关节LCL损伤的分型诊断符合率及踝关节复合损伤检出率。结果:经手术探查或手术修补证实踝关节LCL损伤患者有51例,其中Ⅱ型、Ⅲ型损伤各有20、31例;高频超声与MRI诊断踝关节LCL损伤的敏感度、特异度、准确率、诊断Ⅱ级、Ⅲ级损伤符合率对比,差异无统计学意义(P>0.05);MRI检查骨折检出率高于高频超声,差异有统计学意义(P<0.05);高频超声与MRI检查在距骨关节软骨病变、肌腱损伤、关节腔积液检出率对比,差异无统计学意义(P>0.05)。结论:高频超声诊断踝关节LCL损伤准确率与MRI相当,可显示韧带损伤数量、类型与程度,但高频超声检查具有动态、实时及无创等优点。 Objective:To investigate the diagnostic value of high frequency ultrasound and magnetic resonance imaging(MRI)in the injury of ankle joint lateral collateral ligament(LCL).Method:A total of 93 patients with suspected ankle joint LCL injury treated in our hospital from May 2019 to May 2021 were selected.All patients were examined by ultrasound and MRI.Took the results of surgical exploration or surgical repair as the golden standard,the sensitivity,specificity and accuracy of high-frequency ultrasound and MRI in the diagnosis of ankle joint LCL injury were calculated,and the diagnostic coincidence rate of high-frequency ultrasound and MRI in the classification of ankle joint LCL injury and the detection rate of ankle joint composite injury were compared.Result:A total of 51 cases of ankle joint LCL injury were confirmed by surgical exploration or surgical repair,including 20 cases of typeⅡinjury and 31 cases of typeⅢinjury.There were no significant differences in the sensitivity,specificity,accuracy and the coincidence rate of gradeⅡandⅢinjury of high-frequency ultrasound and MRI in diagnosing ankle joint LCL injury(P>0.05).The fracture detection rate of MRI was higher than that of high-frequency ultrasound,with statistical difference(P<0.05).There were no significant differences between high-frequency ultrasound and MRI in the detection rate of talus articular cartilage lesions,tendon injury and articular cavity fluid(P>0.05).Conclusion:The accuracy of high-frequency ultrasound in the diagnosis of ankle joint LCL injury is similar to that of MRI,which can show the number,type and degree of ligament injury,but high-frequency ultrasound has the advantages of dynamic,real time and non-invasive.
