摘要
目的:探讨左甲状腺素钠片对妊娠期亚临床甲减患者甲状腺功能与妊娠结局的影响。方法:选择2019年2月-2020年6月宁德人民医院收治的84例妊娠期亚临床甲减患者,按照随机数字表法将其分为治疗组(42例)和对照组(42例)。对照组接受常规治疗,治疗组在此基础上口服左甲状腺素钠片治疗。比较两组甲状腺功能、分娩方式、不良妊娠结局发生情况及治疗安全性。结果:治疗后,治疗组促甲状腺素(TSH)水平显著低于对照组,游离甲状腺素(FT_(4))、三碘甲腺原氨酸(FT_(3))水平显著高于对照组,差异均有统计学意义(P<0.05);治疗组剖宫产率低于对照组,差异有统计学意义(P<0.05);治疗组新生儿窒息、早产、产后出血、低体重儿发生率低于对照组,差异均有统计学意义(P<0.05)。两组胎儿畸形发生率比较差异无统计学意义(P>0.05)。两组治疗期间均无明显不良反应发生。结论:左甲状腺素钠片可有效改善妊娠期亚临床甲减患者甲状腺功能,提高自然分娩率,减少不良妊娠结局发生,安全性较高。
Objective:To investigate the effect of Levothyroxine Sodium Tablets on thyroid function and pregnancy outcome in patients with subclinical hypothyroidism during pregnancy.Method:A total of 84 patients with subclinical hypothyroidism during pregnancy admitted in the Ningde People’s Hospital from February 2019 to June 2020 were selected and divided into the treatment group(42 cases)and the control group(42 cases)according to the random number table method.The control group received conventional treatment,and the treatment group received oral Levothyroxine Sodium Tablets on the base of the control group.The thyroid function,delivery methods,occurrence of adverse pregnancy outcome and treatment safety were compared between the two groups.Result:After treatment,the level of thyroid stimulating hormone(TSH)in the treatment group was significantly lower than that in the control group,and the levels of free thyroid(FT_(4))and triiodothyronine(FT_(3))were significantly higher than those in the control group,and the differences were statistically significant(P<0.05).The cesarean section rate in the treatment group was lower than that in the control group,and the difference was statistically significant(P<0.05).The incidence of neonatal asphyxia,premature delivery,postpartum hemorrhage,and low birth weight infants in the treatment group were lower than those in the control group,and the differences were statistically significant(P<0.05).There was no statistical difference in the incidence of fetal malformations between the two groups(P>0.05).There were no obvious adverse reactions occurred in the two groups during the treatment period.Conclusion:Levothyroxine Sodium Tablets can effectively improve the thyroid function of patients with subclinical hypothyroidism during pregnancy,improve the rate of natural delivery,reduce the occurrence of adverse pregnancy outcome,and have high safety.
