摘要 目的:探讨双水平气道正压(BiPAP)通气联合呼吸康复训练对慢性阻塞性肺疾病(COPD)合并肺动脉高压患者肺功能及生物学指标的影响。方法:选取2019年1月-2021年1月笔者所在医院收治的COPD合并肺动脉高压患者86例,按照随机数字表法将其分为两组,各43例。两组均接受常规治疗,对照组在此基础上予以呼吸康复训练治疗,观察组在对照组基础上加用BiPAP通气治疗,两组均持续治疗2周。比较两组肺功能指标、生物学指标、肺动脉压(PAP)、血气分析指标及6 min步行距离。结果:治疗后,观察组FVC、PEF、FEV_(1)/FVC均高于对照组,差异有统计学意义(P<0.05)。治疗后,观察组D-二聚体(D-D)、超敏C反应蛋白(hs-CRP)、PAP均低于对照组,差异有统计学意义(P<0.05)。观察组治疗后PaO_(2)高于对照组,PaCO_(2)低于对照组,6 min步行距离长于对照组,差异有统计学意义(P<0.05)。结论:BiPAP通气联合呼吸康复训练可改善COPD合并肺动脉高压患者肺功能,纠正动脉血气异常,降低D-D、hs-CRP及PAP水平,且有利于提高运动耐力。 Objective:To investigate the effect of BiPAP ventilation combined with respiratory rehabilitation training on pulmonary function and biological indexes in patients with chronic obstructive pulmonary disease(COPD)complicated with pulmonary hypertension.Method:A total of 86 patients with COPD complicated with pulmonary hypertension admitted in our hospital from January 2019 to January 2021 were selected and divided into two groups according to random number table method,with 43 cases in each group.Both groups received routine treatment,and the control group was given respiratory rehabilitation training on this basis,and the observation group was additionally treated with BiPAP ventilation on the basis of the control group.Both groups were treated for 2 weeks.The pulmonary function indexes,biological indexes,pulmonary arterial pressure(PAP),blood gas analysis indexes and 6 min walking distance were compared between the two groups.Result:After treatment,FVC,PEF and FEV1/FVC in the observation group were higher than those in the control group,the differences were statistically significant(P<0.05).After treatment,D-dimer(D-D),hs-CRP and PAP in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).After treatment,PaO2 in the observation group was higher than that in the control group,and PaCO2 was lower than that in the control group,and the 6 min walking distance was longer than that in the control group,the differences were statistically significant(P<0.05).Conclusion:BiPAP ventilation combined with respiratory rehabilitation training can improve pulmonary function,correct arterial blood gas abnormalities,reduce D-D,hs-CRP and PAP levels in patients with COPD complicated with pulmonary hypertension,and improve exercise endurance.
