摘要
目的探讨以Charcot关节为首诊的Chiari畸形(CM)伴脊髓空洞(SM)患者的手术效果及预后。方法回顾性分析2015-01—2018-12于陆军第八十一集团军医院行手术的以Charcot关节病为首诊的5例CM伴SM患者的临床资料。结果5例患者均行小切口微创寰枕减压术+寰枕筋膜松解术,术后随访观察均达1a以上。总有效率80.00%,无明显手术相关并发症,术后相关症状无进展。结论对伴有Charcot关节病的CM伴SM患者应尽早诊断潜在原因,及时手术,阻止疾病进展。小切口微创寰枕减压术+寰枕筋膜松解术是安全、有效的治疗方法。
Objective To explore the postoperative effect and disease progress of Chiari malformation with syringomyelia diagnosed by Charcot joint.Methods A retrospective analysis of 5 cases of Chiari malformation with syringomyelia was made from January 2015 to December 2018 in the army's 81st military hospital,with Charcot joint as the first patient.The effectiveness and disease progress were observed by operation.Results Totally 5 patients were treated with minimally invasive atlanto occipital decompression and atlanto occipital fasciolysis.All patients were followed up for more than one year.The total effective rate was 80.00%.There was no obvious operation-related complications and no progress in postoperative symptoms.Conclusion Chiari malformation with syringomyelia associated with Charcot joint should be diagnosed as early as possible,operated in time to prevent the progress of the disease,and performed minimally invasive atlanto occipital decompression+atlanto occipital fasciolysis surgery is safe and effective.
