摘要
目的:观察熄风通络汤联合卡马西平治疗三叉神经痛的临床疗效。方法:将64例三叉神经痛患者按照随机数字表法分为对照组和观察组,每组各32例。对照组给予卡马西平治疗,观察组在对照组治疗的基础上加用自拟熄风通络汤治疗。比较两组患者的临床疗效、不良反应发生情况及治疗前后中医证候积分、三叉神经痛发作频率、三叉神经痛发作持续时间、疼痛程度、生活质量评分变化情况,检测两组患者治疗前后血浆P物质、β-内啡肽水平变化情况。结果:观察组有效率为96.88%,对照组有效率为81.25%,两组患者有效率比较,差异具有统计学意义(P<0.05)。两组患者治疗后中医证候积分低于本组治疗前,且治疗后观察组低于对照组,差异具有统计学意义(P<0.05)。两组患者治疗后三叉神经痛发作频率、发作持续时间、VAS评分及血浆P物质水平均低于本组治疗前,SF-36评分、β-内啡肽水平高于本组治疗前;治疗后观察组三叉神经痛发作频率、发作持续时间、VAS评分及血浆P物质水平均低于对照组,SF-36评分、β-内啡肽水平高于对照组,差异均有统计学意义(P<0.05)。结论:熄风通络汤联合卡马西平治疗三叉神经痛,可降低三叉神经痛发作频率,缩短发作时间,改善临床症状,提高患者生活质量。
Objective:To observe the clinical curative effect of Wind-Extinguishing and Collateral-Unblocking Decoction combined with carbamazepine on trigeminal neuralgia.Methods:A total of 64 patients with trigeminal neuralgia were randomly divided into the control group and the observation group,with 32 cases in each group.The control group was treated with carbamazepine,and the observation group was treated with self-made Wind-Extinguishing and Collateral-Unblocking Decoction on the basis of the control group.The clinical efficacy,adverse reactions,the changes of TCM syndrome score,trigeminal neuralgia attack frequency,trigeminal neuralgia attack duration,pain degree and quality of life score of the two groups before and after treatment were compared.The changes of the plasma P substance andβ-endorphin test results before and after treatment were tested.Results:The effective rate was 96.88%in the observation group and 81.25%in the control group.There was significant difference between the two groups(P<0.05).The TCM syndrome score of the two groups after treatment was lower than that of the same group before treatment,and that of the observation group after treatment was lower than that of the control group,the difference was statistically significant(P<0.05).After treatment,the attack frequency,attack duration,VAS score and plasma substance P level of trigeminal neuralgia in the two groups were lower than those of the same group before treatment,while SF-36 score,andβ-endorphin level were higher than those of the same group before treatment.After treatment,the attack frequency,duration,VAS score and plasma substance P level of trigeminal neuralgia in the observation group were lower than those in the control group,while SF-36 score,andβ-endorphin level in the control group were significantly higher than those in the control group,and all the differences were statistically significant(P<0.05).Conclusion:Wind-Extinguishing and Collateral-Unblocking Decoction combined with carbamazepine in the treatment of trigeminal neuralgia can reduce the attack frequency of trigeminal neuralgia,shorten the attack time,improve clinical symptoms and the quality of life of patients.
