摘要
目的对比不同入路方式在真菌性上颌窦炎患者鼻内镜手术中的应用效果。方法选取2018年1月至2020年12月于郑州大学第二附属医院南阳路院区接受诊疗的82例真菌性上颌窦炎患者为研究对象,按照随机数表法将其分为A组与B组,各41例。对A组采用经泪前隐窝入路,对B组采用经下鼻甲反转扩大下鼻道入路。观察两组手术临床指标、术后病情控制情况,对比两组术前、手术3个月后窦腔黏膜状态(Lund-Kennedy评分),并统计两组并发症情况。结果B组手术时间及住院时间短于A组,出血量少于A组(P<0.05)。B组术后病情控制情况优于A组(P<0.05)。手术3个月后,两组Lund-Kennedy评分均低于术前,且B组Lund-Kennedy评分低于A组(P<0.05)。A组并发症发生率与B组比较,差异无统计学意义(P>0.05)。结论经下鼻甲反转扩大下鼻道入路手术治疗真菌性上颌窦炎可缩短手术时间及住院时间,减少出血量,改善窦腔黏膜状态,有效控制病情。
Objective To compare the application effect of different approaches in endoscopic sinus surgery in patients with fungal maxillary sinusitis.Methods A total of 82 patients with fungal maxillary sinusitis who received diagnosis and treatment in Nanyang Road Hospital District,the Second Affiliated Hospital of Zhengzhou University from January 2018 to December 2020 were selected as the research objects.According to the random number table method,they were divided into group A and group B,with 41 cases in each group.Group A was treated by the anterior lacrimal recess approach,and group B was treated by the extended inferior nasal approach of inferior turbinate inversion.The surgical clinical indexes and postoperative condition control of the two groups were observed.The sinus mucosa states(Lund-Kennedy scores)before and 3 months after operation were compared between the two groups.The complications of the two groups were counted.Results The operation time and hospitalization time of group B were shorter than those of group A,and the amount of bleeding was less than that of group A(P<0.05).The postoperative condition control of group B was better than that of group A(P<0.05).Three months after operation,the Lund-Kennedy scores of the two groups were lower than those before operation,and the Lund-Kennedy score of group B was lower than that of group A(P<0.05).There was no statistical difference in the incidence of complications between group A and group B(P>0.05).Conclusion The surgical treatment of fungal maxillary sinusitis through the extended inferior nasal approach of inferior turbinate inversion can shorten the operation time and hospitalization time,reduce the amount of bleeding,improve the sinus mucosa states and effectively control the disease.
