摘要
目的探讨围术期抗菌药物管控系统对促进围术期合理使用抗菌药物的作用。方法分别抽取医院2019年第2季度(实施前)和2020年第2季度(实施后)的Ⅰ类切口病历334份和749份。其中,预防性使用抗菌药物的病历分别为105份和210份。对抗菌药物使用目的、预防性使用抗菌药物品种选择、疗程及Ⅰ类切口预防性使用抗菌药物指征进行限制,并对实施效果进行评价。结果实施后,Ⅰ类切口预防性使用抗菌药物的品种选择不合理率由3.81%降至0,用药疗程不合理率由1.90%降至0,预防性抗菌药物使用率由31.44%降至28.04%,全院抗菌药物使用强度由51.45降至43.33。结论应用信息化管控系统对围术期抗菌药物的使用进行干预,能够实现对抗菌药物个性化使用的精准管理,可提高合理用药水平。
Objective To investigate the effect of the control system for perioperative antibiotics on promoting the rational use of perioperative antibiotics.Methods The medical records of typeⅠincision in the hospital in the second quarter of 2019(334 medical records,before the implementation)and the second quarter of 2020(749 medical records,after the implementation)were selected,of which 105 medical records and 210 medical records were the prophylactic use of antibiotics before and after implementation.The use purpose of antibiotics,the selection of varieties of prophylactic antibiotics,the course of treatment and the indications of prophylactic use of antibiotics in typeⅠincision were limited,and the implementation effect was evaluated.Results After the implementation,the unreasonable rate of the variety selection of prophylactic antibiotics for typeⅠincision decreased from 3.81%to 0,the unreasonable rate of treatment course decreased from 1.90%to 0,the use rate of prophylactic antibiotics decreased from 31.44%to 28.04%,and the antibiotics use density(AUD)in the hospital decreased from 51.45 to 43.33.Conclusion The application of information management and control system to intervene the use of perioperative antibiotics can realize the precise management of personalized use of antibiotics and improve the level of rational drug use.
