摘要
目的研究短暂性脑缺血发作(TIA)患者ABCD2评分与外泌体miR-450b-5p水平的关系。方法选取亳州市人民医院2017年1月至2019年6月诊治的142例TIA患者作为TIA组,另选取133例同期于本院体检的健康人作为对照组,检测两组外泌体miR-450b-5p水平,根据ABCD2评分将TIA患者分为低危组、中危组和高危组,比较各组miR-450b-5p水平,分析miR-450b-5p水平与ABCD2评分的相关性。比较不同预后患者ABCD2评分与外泌体miR-450b-5p水平,并通过ROC曲线分析两者对于预后的预测价值。结果 TIA组外泌体miR-450b-5p表达水平明显低于对照组,差异有统计学意义(P<0.05)。ABCD2评分:高危组>中危组>低危组,外泌体miR-450b-5p表达水平:高危组<中危组<低危组,差异均有统计学意义(P<0.05)。外泌体miR-450b-5p水平与ABCD2评分呈负相关(r=-0.611,P<0.05)。预后良好组患者ABCD2评分明显低于预后不良组,外泌体miR-450b-5p水平明显高于预后不良组,差异均有统计学意义(P<0.05)。外泌体miR-450b-5p联合ABCD2评分评估TIA预后的AUC为0.819,明显大于两者单独检测(P<0.05),两者联合预测的敏感度为84.21%,特异度为69.27%。结论 TIA患者外泌体miR-450b-5p表达水平与ABCD2评分呈负相关,两者水平升高增高可提示TIA患者预后不良。
Objective To study the relationship between ABCD2 score and exosomal miR-450b-5p level in patients with transient ischemic attack(TIA).Methods 142 patients with TIA(TIA group) and133 healthy individuals(control group) were enrolled in this study between January 2017 and June 2019.The exosomal miR-450b-5p levels in the two groups were detected.According to the ABCD2 score,patients with TIA were divided into the low-risk group,the moderate-risk group,and the high-risk group.The miR-450b-5p level was compared among the groups,and the correlation between miR-450b-5p level and ABCD2 score was analyzed.The ABCD2 scores and exosomal miR-450b-5p levels of patients with different prognosis were compared.The prognostic value of the two indicators was analyzed by the ROC curve.Results The expression level of exosomal miR-450b-5p in the TIA group was significantly lower than that in the control group(P<0.05).ABCD2 score decreased in order,while the expression level of exosomal miR-450b-5p increased in order of the high-risk group,the moderate-risk group,to the low-risk group(P<0.05).The miR-450b-5p level was negatively correlated with the ABCD2 score(r=-0.611,P<0.05).The ABCD2 score of patients in the good prognosis group was significantly lower than that in the poor prognosis group, while the exosomal miR-450b-5p level was significantly higher than that in the poor prognosis group(P<0.05).The AUC of exosomal miR-450b-5p combined with ABCD2 score to assess the prognosis of TIA was 0.819,which was significantly greater than that of the two alone(P<0.05).The sensitivity and specificity of the two combined prediction were 84.21% and 69.27%.Conclusion The expression level of exosomal miR-450b-5p in TIA patients is negatively correlated with the ABCD2 score.Elevated miR-450b-5p level and ABCD2 score indicate a poor prognosis in TIA patients.
