摘要
目的分析陕西省延安地区女性宫颈人乳头状瘤病毒(HPV)感染情况、特征及基因型分型,为本地区宫颈癌的筛查、防治及HPV感染的流行病学研究提供重要依据。方法总结2018年1月至2020年5月期间在北医三院延安分院(延安市中医医院)进行HPV检测的体检、“两癌”筛查及就诊患者共5472例女性宫颈脱落细胞样本,采用PCR体外扩增和DNA反向斑点杂交法相结合的DNA芯片技术,检测24种HPV核酸亚型(包括18种高危型和6种低危型),分析HPV感染率,并对感染人群年龄分布进行分析。结果5472例检测人群中HPV感染1043例,感染率19.06%。主要为单一高危型感染,感染率为12.02%;单一低危型感染率为2.19%;多重感染HPV率为4.82%。1043例HPV感染者中前五位的高危型分别为:HPV16(12.8%)、HPV52(8.8%)、HPV58(7.6%)、HPV56(4.9%)、HPV51(3.9%);前三位的低危分型分别为HPV42(3.3%)、HPV6(2.2%)、HPV44(2.2%)。1043例感染者按5个年龄组的划分,其中30~39岁年龄组感染阳性占比最高,为40.27%;40~49岁年龄组感染阳性占比次之,为27.52%。结论延安地区妇女HPV感染有其特点,以单一亚型感染为主,最常见的是高危型16、52亚型;30~49岁育龄期妇女是HPV感染的高发人群,应以重视提倡适龄人群大范围接种HPV疫苗,预防宫颈癌及其他HPV相关疾病。并需要定期进行地区及全国范围内的HPV基因分型研究,掌握其流行规律。
Objective To analyze the female cervical human papilloma virus(HPV)infection,characteristics and genotyping in Yan′an,Shaanxi Province,to provide an important basis for the screening,prevention and treatment of cervical cancer and the epidemiological study of HPV infection.Methods Summarize 5472 cases of female cervical exfoliated cell samples from patients undergoing HPV testing,screening for“two cancers”and treating patients in Yan′an Branch of the Third Hospital of Beijing Medical University(Yan′an Traditional Chinese Medicine Hospital)from January 2018 to May 2020.Using DNA chip technology combining PCR in vitro amplification and DNA reverse dot hybridization to detect 24 human papillomavirus nucleic acid subtypes(including 18 high-risk types and 6 low-risk types),analyze the HPV infection rate,and analyze the infection rate.The age distribution of the population is analyzed.Results There were 1043 HPV infections in the 5472 tested population,and the infection rate was 19.06%.It was mainly a single high-risk infection,with an infection rate of 12.02%;a single low-risk infection rate was 2.19%;and a multiple infection HPV rate was 4.82%.The top five high-risk types among 1043 cases of HPV infection were:HPV16(12.8%),HPV 52(8.8%),HPV 58(7.6%),HPV 56(4.9%),HPV 51(3.9%);the three low-risk types were HPV42(3.3%),HPV 6(2.2%),and HPV 44(2.2%).1043 cases of infection were divided into 5 age groups.Among them,the 30~39-year-old age group had the highest proportion of infections at 40.27%;the 40~49-year-old age group had the second highest proportion of infections,at 27.52%.Conclusion The HPV infection of women in Yan′an area has its own characteristics.The main infection is single subtype,and the most common are high-risk subtypes 16 and 52.Women of childbearing age between 30 and 49 years old are the population with a high incidence of HPV infection,and attention should be paid to promoting HPV vaccination among the age-appropriate population to prevent cervical cancer and other HPV-related diseases,and regular regional and national HPV genotyping studies should be carried out.,Master its popular law.
