摘要
天地转气汤是袁梦石主任医师所创制的主要用于治疗阿尔茨海默病中晚期证属肾精亏虚、痰浊蒙窍的个人经验方。该文旨在从对阿尔茨海默病(痴呆病)的中医病因病机、病理特点的认识及组方用药的思路,着手总结袁梦石老师使用该方的临床经验和理论特色,并附典型医案2例。袁师认为,阳衰阴盛是阿尔茨海默病的基本病因病机;肝血亏虚、肝气郁结的体质者易患阿尔茨海默病;从肾精亏虚到痰浊蒙窍是阿尔茨海默病发展的不同阶段;阿尔茨海默病中晚期的证型往往多种证候兼夹并见,但临床特点有较多的一致性;天地转气汤主要针对阿尔茨海默病中晚期的常见证候特点所设,适用于大多数这类患者。
Tiandi Zhuanqi decoction is a personal experience formula created by Chief Physician Yuan Mengshi,which is mainly used to treat the middle and late stages of Alzheimer’s disease with deficiency of kidney essence and turbid phlegm.This article aims to summarize the clinical experience and theoretical characteristics of Ms.Yuan Mengshi’s use of the prescription from the understanding of the etiology,pathogenesis and pathological characteristics of Alzheimer’s disease (dementia) and the thinking of formulating the prescription,and attaching a typical medical record.2 cases.Master Yuan believes that yang decay and yin prosperity are the basic etiology and pathogenesis of Alzheimer’s disease;people with liver-blood deficiency and stagnated liver-qi are prone to Alzheimer’s disease;from deficiency of kidney essence to turbid orifice of phlegm is Alzheimer’s disease.Different stages of the development of Alzheimer’s disease;the middle and late stages of Alzheimer’s disease are often mixed with multiple syndromes,but the clinical characteristics are more consistent;Tiandi Zhuanqi decoction is mainly aimed at Alzheimer’s disease Based on the characteristics of the common syndromes in the late stage,it is suitable for most of these patients.
