摘要
目的探讨早期妊娠患者血清白细胞介素-17(IL-17)、血管抑素(AS)、血管生成素(Ang)检测在稽留流产诊断中的应用价值。方法将2018年6月至2019年11月华中科技大学同济医学院附属同济医院收治的37例早期流产患者纳入早期流产组,41例稽留流产患者纳入稽留流产组,另选取体检健康妊娠产妇30例为对照组。分析比较3组对象血清IL-17、AS、Ang水平;采用Pearson相关性分析稽留流产患者血清IL-17、AS、Ang间的关系;采用受试者工作特征(ROC)曲线分析血清IL-17、AS、Ang水平妊娠早期诊断稽留流产的价值。结果对照组、早期流产组、稽留流产组血清IL-17、AS水平逐渐升高,血清Ang-1、Ang-2水平逐渐降低,差异均有统计学意义(P<0.05);IL-17水平与AS呈正相关(r=0.872,P<0.05),与Ang-1、Ang-2呈负相关(r=-0.824、-0.796,P<0.05);AS与Ang-1、Ang-2呈负相关(r=-0.813、-0.776,P<0.05);Ang-1与Ang-2呈正相关(r=0.770,P<0.05);经ROC曲线分析,IL-17取值29.13时,妊娠早期诊断稽留流产的曲线下面积(AUC)为0.719;AS取值8.08时,妊娠早期诊断稽留流产的AUC为0.729;Ang-1取值5.66时,妊娠早期诊断稽留流产的AUC为0.842;Ang-2取值4.70时,妊娠早期诊断稽留流产的AUC为0.851;四项指标联合妊娠早期诊断稽留流产的AUC为0.936。结论早期妊娠患者血清IL-17、AS、Ang-1、Ang-2联合检测在稽留流产的诊断中具有较高价值,值得临床推广使用。
Objective To investigate the value of serum interleukin-17(IL-17),angiostatin(AS)and angiopoietin(Ang)detection in the diagnosis of missed abortion in early pregnancy.Methods Thirty-seven cases of early abortion(early abortion group),41 cases of missed abortion(missed abortion group)and 30 cases of healthy pregnant women(control group)in Tongji Hospital,Tongji Medical College of HUST from June 2018 to November 2019 were collected.Levels of serum IL-17,AS and Ang were compared among three groups.Receiver operating characteristic(ROC)curve was used to analyze the value of serum IL-17,AS and Ang levels in diagnosis of missed abortion in early pregnancy.Results The levels of serum IL-17 and AS among three groups were the lowest in control group,followed by early abortion group,and were the highest in missed abortion group,the differences were statistically significant(P<0.05).Serum Ang-1 and Ang-2 among three groups were the lowest in missed abortion group,followed by early abortion group,and were the highest in control group,the differences were statistically significant(P<0.05).IL-17 level was positively correlated with AS(r=0.872,P<0.05),and negatively correlated with Ang-1 and Ang-2(r=-0.824,-0.796,P<0.05);AS was negatively correlated with Ang-1 and Ang-2(r=-0.813,-0.776,P<0.05);Ang-1 level was positively correlated with Ang-2(r=0.770,P<0.05).The area under the curve(AUC)of IL-17,AS,Ang-1 and Ang-2 in the diagnosis of missed abortion in early pregnancy were 0.719,0.729,0.842 and 0.851 when their values were 29.13,8.08,5.66 and 4.70,respectively.AUC of combined detection of four indexes in the early diagnosis of missed abortion was 0.936.Conclusion Combined detection of serum IL-17,AS,Ang-1 and Ang-2 is of high value in the diagnosis of missed abortion in early pregnancy,and is worthy of clinical application.
