摘要
目的观察新生儿缺氧缺血性脑病(HIE)患儿磁共振(MR)波谱表现,探讨脑苷肌肽的治疗效果,分析其对患儿神经元特异性烯醇化酶(NSE)、肿瘤坏死因子-α(TNF-α)水平的影响。方法40例HIE患儿,采用随机数字表法分为研究组和对照组,各20例。对照组给予脑蛋白水解物注射液治疗,研究组在对照组的基础上增加脑苷肌肽注射液治疗。观察患儿头部MR波谱变化,比较两组患儿的临床神经症状、肌张力、原始反射恢复时间,治疗前后的新生儿行为神经评分表(NBNA)评分、血清NSE、TNF-α水平。结果HIE患儿MR波谱多表现为梗死、缺血、脑水肿,部分患儿可并发脑出血。治疗后,研究组患儿的临床神经症状、肌张力、原始反射恢复时间分别为(3.28±1.03)、(6.12±1.54)、(4.89±1.17)d,均短于对照组的(4.11±1.31)、(7.04±1.30)、(6.02±1.76)d,差异具有统计学意义(P<0.05)。治疗后,两组患儿的NBNA评分明显高于本组治疗前,且研究组患儿的NBNA评分(30.50±2.50)分明显高于对照组的(25.50±2.81)分,差异具有统计学意义(P<0.05)。治疗后,两组患儿的血清NSE、TNF-α水平明显低于本组治疗前,且研究组患儿的血清NSE(22.70±4.30)μg/L、TNF-α(21.10±3.98)ng/L明显低于对照组的(27.79±5.38)μg/L、(33.38±4.46)ng/L,差异具有统计学意义(P<0.05)。结论HIE患儿的MR波谱表现异常,脑苷肌肽可明显改善HIE患儿的临床神经症状,缓解机体炎症反应,促进神经功能恢复。
Objective To observe the magnetic resonance(MR)spectrum performance in neonates with hypoxic ischemic encephalopathy(HIE),discuss the therapeutic effect of cattle encephalon glycoside and ignotin,and analyze its influence on the levels of neuron-specific enolase(NSE)and tumor necrosis factor-α(TNF-α).Methods A total of 40 neonates with HIE were divided into research group and control group according to the random numerical table,with 20 cases in each group.The control group was treated with cerebroprotein hydrolysate injection,while the research group was treated with cattle encephalon glycoside and ignotin injection on the basis of the control group.The changes in the MR spectrum of the head of the child was observed,and the clinical neurological symptoms,muscle tension,recovery time of original reflexes,neonatal behavioral neurological assessment(NBNA)scores,serum NSE and TNF-αlevels before and after treatment were compared between the two groups.Results MR spectroscopy in neonates with HIE mostly manifested as infarction,ischemia,and cerebral edema,and some may be complicated by cerebral hemorrhage.After treatment,the clinical neurological symptoms,muscle tension and recovery time of original reflexes of the research group were(3.28±1.03),(6.12±1.54)and(4.89±1.17)d,respectively,which were shorter than(4.11±1.31),(7.04±1.30)and(6.02±1.76)d of the control group,and the differences were statistically significant(P<0.05).After treatment,NBNA score of the two groups were significantly higher than those of this group before treatment,and NBNA scores(30.50±2.50)points of the research group were significantly higher than(25.50±2.81)points of the control group.All the differences were statistically significant(P<0.05).After treatment,the serum NSE and TNF-αlevels of the two groups were significantly lower than those before treatment,and the serum NSE(22.70±4.30)μg/L and TNF-α(21.10±3.98)ng/L of the research group were significantly lower than(27.79±5.38)μg/L and(33.38±4.46)ng/L of the control group.All the differences were statistically significant(P<0.05).Conclusion The MR spectrum is abnormal in neonates with HIE,cattle encephalon glycoside and ignotin can significantly improve the clinical neurological symptoms of neonates,relieve the inflammatory reaction and promote the recovery of neurological function.
