摘要
目的:探讨黄柏对高尿酸血症慢性肾脏病(CKD)大鼠的心脏保护作用及RAS系统在其中的作用。方法:雄性SD大鼠30只随机分三组:a组(CKD组),b组(高尿酸血症CKD组),c组(黄柏组)。b、c组使用氧嗪酸钾灌胃建立高尿酸血症CKD模型,c组并使用黄柏灌胃。24周后处死大鼠,检测血清肾功能指标及AngⅡ,镜下观察大鼠心肌病变,检测心脏Ⅰ型胶原及AngⅡ的表达。结果:(1)三组大鼠血肌酐水平差异无统计学意义,肾脏病理均未见明显病理改变,b、c组血尿酸水平显著高于a组,其中c组显著低于b组。(2)AngⅡ水平b、c组血显著高于a组,其中c组显著低于b组。(3)a组未见明显心脏病变,b、c组出现心脏病理改变,且b组损伤程度高于c组。b、c组心脏Ⅰ型胶原的表达显著高于a组,其中c组显著低于b组。b、c组AngⅡ在心脏的表达显著高于a组,其中c组显著低于b组。(4)大鼠血尿酸水平与血AngⅡ水平及其在心脏组织中的表达、Ⅰ型胶原在心脏组织中的表达呈显著正相关,Ⅰ型胶原在心脏组织中的表达亦与血AngⅡ水平及其在心脏组织中的表达呈显著正相关。结论:黄柏通过其降低血尿酸水平及阻断RAS系统活性的作用对伴高尿酸血症CKD大鼠心肌损伤具有保护作用。
Objective:To explore the cardiac protective effect of the Chinese traditional medicine phellodendri in CKD rats with hyperuricemia and the role of RAS system.Methods:30 male SD rats were randomly divided into three groups:group a(CKD group),group b(CKD with hyperuricemia group),group c(phellodendri intervention group).We established the hyperuricemia CKD animal model by right-side nephrectomy and feeding them with potassium oxonate.We fed the rats in group c with phellodendri.We detected the serum indexes of UA、Scr and AngⅡ.The expressions of collagenⅠand AngⅡin heart tissue were detected.The pathological damage of heart was observed using photoscope and electron microscope.Results:There were no obvious pathological changes in kidney of all the rats,and no obvious difference in Scr among the 3 groups.The levels of UA in group b,c were significantly higher than that of group a,and the levels of serum UA in group c were significantly lower than that of group b.The levels of serum AngⅡin group b,c were significantly higher than that of group a,the levels of serum AngⅡin group c were significantly lower than that of group b.The expression of AngⅡin heart tissue showed the same.The expression of collagen I in group b,c were significantly higher than that of group a,its expressions in group c were significantly lower than that of group b.There were obvious cardiac lesions in group b,showing myocardial cell hypertrophy,arrangement disorder,and interstitial fibrosis,and the microstructure of myocardial cells showed significant damage.The pathological changes of heart tissure in group c were similar to that of group b,but more mild.Linear regression analysis showed that the levels of serum UA had significant positive correlations with the levels of AngⅡand its expressions in heart tissure,and the expressions of collagen I.Linear regression analysis also showed that the expressions of collagen I had significant positive correlations with the levels of AngⅡand its expressions in heart tissure.Conclusion:The Chinese traditional medicine phellodendri has a protective effect on heart damage in CKD rats with hyperuricemia,and may be related to the reduction of serum uric acid and activity of RAS system.
