摘要
目的探究内镜下等离子刀治疗鼻咽癌放疗后原发灶残留的临床效果。方法选择放疗后原发灶残留患者94例作为研究对象,根据手术方式不同分为2组,对照组行内镜下鼻咽原发病灶残留切除术,观察组行内镜下等离子刀原发病灶残留切除术。分析手术情况和随访情况,比较2组患者生活质量。结果对照组手术时间明显长于观察组(P<0.05)、出血量明显多于观察组(P<0.05),2组住院时间、并发症发生率无明显差异(P>0.05)。对照组躯体、认知、角色、情绪和社会功能得分明显低于观察组,总体健康状况明显低于观察组(P<0.05)。2组1年生存率和无瘤生存率均为100%。结论内镜下等离子刀治疗鼻咽癌放射治疗后原发灶残留,近期临床效果较好,无严重并发症发生,且患者生活质量改善情况更好。
Objective To explore the clinical effect of endoscopic plasma knife treatment on residual primary tumors of nasopharyngeal carcinoma after radiotherapy.Methods 94 patients with residual primary lesions after radiotherapy were selected,and divided into 2 groups according to different treatment methods.The control group underwent endoscopic residual resection of nasopharyngeal primary lesions,and the observation group underwent endoscopic plasma knife residual resection of primary lesions.Analyzed the status of surgery and follow-up,and compare the quality of life of patients before and after surgery.Results The operation time of the control group was significantly longer than that of the observation group(P<0.05),and the blood loss was significantly higher than that of the observation group(P<0.05).The length of hospital stay in the two groups was similar,and there was no significant difference in the incidence of complications(P>0.05).The physical,cognitive,role,emotional and social function scores of the control group were significantly lower than those of the observation group,and the overall health status was significantly lower than that of the observation group.The one-year survival rate and tumor-free survival rate of both groups were 100%.Conclusion After endoscopic plasma knife treatment of nasopharyngeal carcinoma with radiotherapy,the primary tumor remains,the short-term clinical effect is better,without serious complications,and the patient's quality of life is better.
