摘要
目的观察宫颈癌患者的血浆循环肿瘤DNA(circulating-tumor DNA,ctDNA)及染色体拷贝数变异(copy number variations,CNVs),探讨与宫颈癌临床病理特征的相关性。方法选取2017年3月至2017年10月在青岛市市立医院就诊的宫颈癌患者30例,收集治疗前外周血标本,应用二代测序技术(next generation sequencing,NGS)检测ctDNA及CNVs,分析患者的临床资料及病理特征,评估染色体拷贝数变异情况及ctDNA与临床病理特征的关系。结果30例患者中,ctDNA阳性率33.3%(10/30),ctDNA与宫颈癌的分期、肿瘤大小及间质浸润有关(P<0.05),而与年龄、HPV感染、SCC水平、病理类型、组织学分级、宫旁浸润、淋巴结转移、手术切缘及淋巴脉管间隙浸润无关(P>0.05)。CNVs发生率为46.7%(14/30),最常见的为3号及6号染色体,14例发生拷贝数变异的患者中,3号染色体占50%(7/14),3q扩增占35.7%(5/14),6号染色体占35.7%(5/14)。结论临床分期为ⅡA期,肿瘤大小>2 cm,间质浸润深度>1/2的宫颈癌患者血浆ctDNA检出率明显升高;CNVs最常见于3号及6号染色体,其中3q扩增最常见,ctDNA及CNVs在宫颈癌的临床诊治中有一定的价值。
Objective To observe the plasma circulating-tumor DNA(ctDNA)and chromosome copy number variations(CNVs)in patients with cervical cancer,and explore the correlation of plasma ctDNA and CNVs with the clinicopathological characteristics of these patients.Methods Clinical data of 30 patients with cervical cancer who were treated at Qingdao Municipal Hospital from March 2017 to October 2017 were retrospectively analyzed.Peripheral blood samples were collected before treatment.CtDNA and CNVs were detected using next generation sequencing(NGS)technology to analyze their relationship with the clinicopathological characteristics of patients with cervical cancer.Results Among the 30 patients,the positive rate of ctDNA was 33.3%(10/30).ctDNA was correlated with the stage,tumor size and interstitial infiltration of cervical cancer(P<0.05),but not with age,HPV infection,SCC level,pathological type,histological grade,lymph node metastasis,parametrial infiltration,surgical margin and lymphoma vascular space invasion(P>0.05).The incidence of CNVs,most common on chromosomes 3 and 6,was 46.7%(14/30).Among the chromosomes,chromosome 3 accounted for 50%(7/14),3q amplification for 35.7%(5/14),and chromosome 6 for 35.7%(5/14).Conclusion There is a significantly higher positive detection rate of ctDNA in cervical cancer patients with clinical stageⅡA,tumor size>2 cm,and interstitial infiltration depth>1/2.CNVs,of which,3q amplification is the most common,usually occur on chromosomes 3 and 6.CtDNA and CNVs have certain value in the clinical diagnosis and treatment of cervical cancer.
