摘要
目的:探讨不同胎龄早产儿振幅整合脑电图(aEEG)的变化规律及其与脑损伤的关系。方法:回顾性分析我院新生儿科收治的87例早产儿临床资料,依据早产儿是否发生脑损伤分为脑损伤组、无脑损伤组,分析不同胎龄早产儿aEEG变化规律,明确aEEG变化规律与脑损伤的关系。结果:随胎龄增加,早产儿aEEG的图形不连续电压明显减少,而连续性电压和aEEG图形周期性显著增高(P<0.05);随胎龄增加,睡眠-觉醒周期出现百分比增加,34周及以后出生早产儿几乎全部出现成熟的睡眠-觉醒周期,其中一次睡眠觉醒周期平均时程约为87~102min,差异无统计学意义(P>0.05)。随胎龄增加,宽带和窄带的带宽均有逐渐变窄的趋势,其中窄带下界电压之间比较差异有统计学意义(P<0.05)。随胎龄增加早产儿aEEG成熟度总评分逐步增高(P<0.05)。脑损伤组胎龄<34周、aEEG为不连续低电压的占比明显高于无脑损伤组,而窄带下界电压、aEEG总评分较无脑损伤组明显低(P<0.05)。胎龄、aEEG变化规律(图形的连续性和周期性、窄带下界电压、aEEG总评分)是早产儿脑损伤的风险性因素(P<0.05)。结论:不同胎龄早产儿aEEG变化特征存在差异,其中胎龄越小、aEEG为不连续性低电压且窄带下界电压和aEEG总评分低的早产儿脑损伤发生的风险相对较高。
Objective:To investigate the change law of amplitude integrated electroencephalogram(aEEG)in premature infants of different gestational age,and its relationship with brain injury.Methods:This study retrospectively analyzed the clinical data of 87 premature infants admited to neonatology department of the hospital.The premature infants were divided into brain injury group and non-brain injury group according to the presence or absence of brain injury.Changes of aEEG in premature infants of different gestational age were analyzed,and their relationship with brain injury was determined.Results:With the increase of gestational age,the discontinuous voltage on aEEG decreased significantly,while continuous voltage and aEEG significantly increased periodically(P<0.05).With the increase of gestational age,the percentage of sleep-waking cycles increased.Almost all premature infants born at 34 weeks and later had mature sleep-waking cycles,and the average duration of a sleep-waking cycle was about 87~102 minutes(P>0.05).With the increase of gestational age,the bandwidths of both broadband and narrowband tended to narrow,and significant differences were observed,compared with the lower bound narrowband voltage(P<0.05).With the increase of gestational age,the total score of aEEG maturity of premature infants increased(P<0.05).The proportions of gestational age<34 weeks and discontinuous low voltage on aEEG were significantly higher in the brain injury group than in the non-brain injury group,while the narrowband voltage and total aEEG score were significantly lower than in the non-brain injury group(P<0.05).Gestational age and aEEG change law(graph continuity and periodicity,narrowband voltage,total aEEG score)were risk factors for brain injury in premature infants(P<0.05).Conclusion:Changes in aEEG are different in premature infants of different gestational age.The smaller the gestational age,the higher the risk of brain injury in premature infants with discontinuous low voltage on aEEG and low narrowband voltage and total aEEG scores.
