摘要
右美沙芬(dextromethorphan,DM)是一种N·甲基-D-天冬氨酸(NMDA)受体拮抗药,具有提高脑内五羟色胺(5-HT)等神经递质的作用,小剂量时可改善情绪,而大剂量时可引发严重的精神症状。我院收治的1例右美沙芬超量所致的严重兴奋、话多、行为冲动合并有幻听幻视、夸大妄想和被害妄想的中毒患者,使用大剂量抗精神病药物和情绪稳定剂治疗后病情仍难以控制,一度病情减轻后又反复发作,最后经1个多月治疗才基本好转,现将该病例报道如下,以期为临床治疗提供参考。
Dextromethorphan(DM)is a receptor antagonist of N·Methyl-D-aspartic acid(NMDA),which can improve the effect of neurotransmitters such as serotonin(5-HT)in the brain.Small dose of DM can improve mood,while large dose of DM may cause severe psychiatric symptoms.A case study of our hospital shows that excess does of DM can cause a series of symptoms of the patient,such as severe excitement,talkativeness,impulsive behavior,auditory hallucinations,visual hallucination,grandiose delusions and persecutory delusions,after high-dose antipsychotics and mood stabilizers treatment,the condition is still difficult to control with periodic remission and repeated attacks reciprocally.Finally,it is basically improved after more than one month of treatment.The case is reported as follows,in order to provide reference for clinical treatment.
