摘要
目的:分析无精子症患者的外周血染色体核型与血清生殖激素水平的关系。方法:选取2016年1月~2019年10月份间在我院男科门诊进行就诊的986例无精子症患者为病例组,另选同期在本院进行精液检查的2020例正常男性为对照组,两组均进行外周血染色体核型分析和血清生殖激素水平检测。结果:986例无精子症患者中异常染色体核型检出232例,异常核型检出率为23.53%;2020例对照组中异常染色体核型检出51例,异常核型检出率为2.52%,两组具有显著性差异(P<0.01)。无精子症组中性染色体异常核型检出213例,占染色体异常核型总数的91.81%,其中核型47,XXY最常见,共检出162例,占异常核型总数的69.83%;常染色体异常核型10例,占异常核型总数的4.31%;另外,检出性反转8例,占异常核型总数的3.45%;核型47,XY,+mar检出1例,占异常核型总数的0.43%。无精子症组的FSH、LH和PRL水平明显高于对照组,差异显著(P<0.01),T水平明显低于对照组,差异显著(P<0.01),两组E2水平无显著差异(P>0.05)。但无精子症组中异常核型的E2水平低于对照组,且两组具有显著差异(P<0.01)。结论:无精子症与染色体核型异常和生殖激素水平有密切关系。
Objective:To analyze the peripheral blood karyotype and the relationship with serum reproductive homone levels in patients with azoospermia.Methods:986 azoospermia patients who visited our male outpatient clinic between January 2016 and October 2019 were selected as the case group,and another 2020 normal men who underwent semen examination in our hospital during the same period were selected as the control group.Analyses of peripheral blood karyotype and serum reproductive hormone levels were performed in both groups.Results:Among 986 azoospermia patients,232 cases of abnormal karyotype were detected,and the abnommal karyotype detection rate was 23.53%;among 2020 control cases,51 cases of abnommal karyotype were detected,and the abnommal karyotype detection rate was 252%,with significant differences between the two groups(P<0.01).In the azoospermia group,213 cases of abnormal neutrophilic karyotypes were detected,accounting for 91.81%of the total number of abnormal karyotypes,among which karyotype 47,XXY was the most common,with 162 cases detected,accounting for 69.83%of the total number of abnormal karyotypes,10 cases of abnommal autosomal karyotypes,accounting for 4.31%of the total number of abnormal karyotypes,in addition,8 cases of sexual inversion were detected,accounting for 3.45%of the total number of abnormal karyotypes;The karyotype 47,XY,+mar was detected in 1 case,accounting for 0.43%of the total abnormal karyotypes The levels of FSH,LH and PRL in the azoospemia group were significantly higher than those in the control group,with significant differences(P<0.01),and the 1evels of T were significantly lower than those in the control group,with significant differences(P<0.01),and there was no significant difference in the levels of E2 between the two groups(P>0.05).However,the E2 level in the abnormal karyotype group in azoospermia was lower than that in the control group,and there was a significant difference between the two groups(P<0.01).Conclusion:Azoospermia is closely related to abnormal karyotype and reproductive hormone levels.
