某采供血机构三次不同类型突发事件血液保障情况回顾性分析 被引量:4

摘要 目的回顾性分析某采供血机构三次重大突发事件血液保障数据的特点,为突发事件血液保障体系建立提供参考。方法通过统计分析某采供血机构在"5·12"汶川地震(A组)、成都"6·5"公交车燃烧(B组)、2020新冠疫情期间(C组)的血液采供数据,总结不同类型突发事件伤员的用血特点。结果 1. A、B、C三组伤病员主要用血原因为外伤/烧伤/血液及消化系统等内科疾病;2.人均输注全血0.00/9.99/0.00 U、悬浮红细胞7.10/14.19/5.20 U、血浆16.86/92.98/11.98 U、血小板0.20/3.07/1.22治疗量、冷沉淀1.00/2.10/4.20 U;3.三组血液来源构成比(自采/调剂)为0.71/1.98/1.47,差异具有统计学意义(P<0.05);4.三组因过期报废红细胞50.00/0.00/59.50 U。结论三次突发事件救治期间的血液保障均与常规医疗救治的血液保障差异大。主要用血原因均不同,但主要使用的成分都是红细胞和血浆,且血液调剂在每次突发事件的血液保障中均发挥了重要作用。因此,一定量的实物红细胞、血浆常规储备可作为做好突发事件血液保障的基础;区域血液联动调配机制的建立对保证突发事件血液保障能力至关重要。 Objective To retrospectively analyze the characteristics of the blood supportability data of three major emergencies in a blood collection and supply institution and provide references for the establishment of the blood supportability system for emergencies. Methods Statistical analysis was carried out on the blood data from a blood collection and supply institution during the "5.12" Wenchuan earthquake(group A), Chengdu "6.5" bus burning(group B), and the 2020 COVID-19 pandemic(group C). The transfusion characteristics of the wounded in the incidents were summarized. Results 1. The main cause of transfusion was trauma(group A), burns(group B), blood and digestive diseases and other internal medical diseases(group C);2. Per capita transfusion of the whole blood 0.00/9.99/0.00 U, suspended red blood cells 7.10/14.19/5.20 U, plasma 16.86/92.98/11.98 U, platelet 0.20/3.07/1.22 therapeutic dose, and cryoprecipitate 1.00/2.10/4.20 U;3. The blood source composition ratio(self-collection/reallocation) of the three groups were 0.71/1.98/1.47 respectively, the differences were statistically significant(P<0.05);4. The expired of red blood cells in the three groups was 50.00/0.00/59.50 U respectively. Conclusion The blood supportability during the three emergencies are different from those in conventional medical treatments. In the face of different reasons for transfusion, the main blood components used remain red blood cells and plasma, and blood reallocation is vital in the blood supportability of each emergency. Therefore, for blood supportability in emergencies, the routine reserve of a certain amount of physical red blood cells and plasma serves as the foundation and the establishment of a regional blood allocation mechanism is essential.
