摘要
股骨颈骨折是临床上多发的骨折类型之一,其发生率占成人全身骨折总数的3.58%,最近的流行病学调查显示,老年人股骨颈骨折的发生率是全身骨折总发生率的50%左右,女性多见。老年股骨颈骨折的发生是多种原因综合作用的结果,其发生与股骨颈生理结构的特殊性、老年人的生理特点及老年人易患多种基础疾病等因素密不可分。老年人多合并心脑血管、呼吸等多系统基础疾病,抵抗力差,骨折后长期卧床易发生感染、压力性损伤、坠积性肺炎等多种严重的骨折并发症,失治、误治常会导致严重的后果,甚至危及患者生命。老年股骨颈骨折的治疗方法主要包括保守治疗和手术治疗两种方式,当前学术界的观点是老年股骨颈骨折应该选择合适的手术方式尽快予以手术治疗,以期达到延长老年人生存率、提高生活质量的目的。本文就老年股骨颈骨折的手术治疗进展进行了综述。
Femoral neck fracture is one of the most common types of fractures in the clinic, whose incidence rate accounts for 3.58% of the total adult systemic fractures. Recent epidemiological survey shows that the incidence of femoral neck fracture in the elderly is about 50% of the total incidence of systemic fractures, and is more common in women. The occurrence of femoral neck fracture in the elderly is the result of the combination of many reasons, which is closely related to the special physiological structure of the femoral neck, the physiological characteristics of the elderly and their susceptibility to various basic diseases. Elderly people have poor immune systems and are prone to complications with cardiovascular, cerebrovascular, respiratory and other multi-system diseases. Long-term bed rest after fracture could lead to infection, bedsores, pendulous pneumonia and other serious fracture complications, while abandoning therapy and misdiagnosis often cause serious consequences and even endanger the patients’ lives. The treatment of senile femoral neck fracture mainly includes conservative treatment and surgical treatment. The current academic view suggests the senile femoral neck fracture patients be treated as soon as possible with the appropriate surgical method, in order to prolong the survival rate and improve the quality of life. This article reviews the progress of the surgical treatment on senile femoral neck fracture.
