多参数磁共振成像技术诊断前列腺癌的价值分析 被引量:2

摘要 目的分析多参数磁共振成像技术(MP-MRI)诊断前列腺癌的价值。方法123例疑似前列腺癌患者,均行多参数磁共振成像技术扫描检查,以病理组织检查结果为金标准,分析多参数磁共振成像技术诊断前列腺癌的符合率、灵敏度、特异度;对比前列腺良性病变、前列腺癌患者的表观弥散系数值(ADC)、胆碱(Cho)+肌酸氢质子磁共振波谱值(Cr)/枸橼酸盐氢质子磁共振波谱值(Cit)(CC/C)、容量转移常数(Ktrans)、速率常数(Kep)、血管外细胞外容积分数(Ve)、初始曲线下面积(iAUC)。结果经过病理组织检查,123例患者中38例属于良性病变,85例属于恶性病变。经多参数磁共振成像技术检查,123例患者中39例属于良性病变,84例属于恶性病变,其中良性确诊37例,恶性确诊83例,诊断符合率为97.56%(120/123);漏诊2例,灵敏度为97.65%(83/85);误诊1例,特异度为97.37%(37/38)。前列腺癌患者的ADC(0.60±0.05)×10^(-3) mm^(2)/s显著低于前列腺良性病变患者的(0.98±0.09)×10^(-3) mm^(2)/s,CC/C(2.28±0.18)、Ktrans(0.30±0.03)/min、Kep(0.73±0.06)/min、Ve(0.41±0.04)、iAUC(36.82±3.25)mmol/(L·min)显著高于前列腺良性病变患者的(0.79±0.07)、(0.11±0.01)/min、(0.41±0.04)/min、(0.26±0.02)、(14.22±1.07)mmol/(L·min),差异具有统计学意义(P<0.05)。结论多参数磁共振成像技术诊断前列腺癌的灵敏度、特异度高,可以较好地区分良性、恶性病变,具有较高的临床应用价值。 Objective To analyze the value of multi-parameter magnetic resonance imaging(MP-MRI)in the diagnosis of prostate cancer.Methods There were 123 patients with suspected prostate cancer,and they all underwent multi-parameter magnetic resonance imaging scan.Taking the results of pathological tissue examination as the gold standard,the coincidence rate,sensitivity,and specificity of multi-parameter magnetic resonance imaging in the diagnosis of prostate cancer were analyzed.The apparent diffusion coefficient(ADC),choline(Cho)+creatine(Cr)/citrate(Cit)(CC/C),volume transfer constant(Ktrans),rate constant(Kep),extravascular extracellular volume fraction(Ve),initial area under the curve(iAUC)of patients with benign prostatic lesions and prostate cancer were compared.Results After pathological examination,38 cases had benign lesions and 85 cases had malignant lesions.According to the multi-parameter magnetic resonance imaging,39 cases had benign lesions and 84 cases had malignant lesions of the 123 patients.Among them,37 cases were benign and 83 cases were malignant,with the diagnosis coincidence rate as 97.56%(120/123),2 cases were missed,with the sensitivity as 97.65%(83/85),1 case was misdiagnosed,with the specificity as 97.37%(37/38).The ADC(0.60±0.05)×10^(-3) mm^(2)/s of patients with prostate cancer was significantly lower than(0.98±0.09)×10^(-3) mm^(2)/s of patients with benign prostate lesions,and the CC/C(2.28±0.18),Ktrans(0.30±0.03)/min,Kep(0.73±0.06)/min,Ve(0.41±0.04),iAUC(36.82±3.25)mmol/(L·min)were significantly higher than(0.79±0.07),(0.11±0.01)/min,(0.41±0.04)/min,(0.26±0.02),(14.22±1.07)mmol/(L·min)of patients with benign prostate disease.All the difference was statistically significant(P<0.05).Conclusion Multi-parameter magnetic resonance imaging has high sensitivity and specificity in the diagnosis of prostate cancer,which can better distinguish benign and malignant lesions,and has high clinical application value.
