摘要
目的探讨血清甲胎蛋白(AFP)联合癌胚抗原(CEA)检测对肝癌的诊断价值。方法将160例肝癌患者作为观察组,再将同时期160例健康体检者作为对照组,检测两组患者的血清AFP、CEA水平和肝功能情况。比较不同肝功能、肿瘤直径和临床分期肝癌患者血清AFP、CEA水平,分析血清AFP联合CEA检查的诊断效能。结果观察组患者AFP、CEA表达水平均明显高于对照组(P﹤0.01),观察组中肝功能异常患者AFP、CEA表达水平均明显高于肝功能正常患者(P﹤0.01)。肿瘤直径≥5 cm患者血清AFP、CEA表达水平均明显高于肿瘤直径﹤5 cm患者(P﹤0.01)。肿瘤分期为Ⅲ~Ⅳ期患者血清AFP、CEA表达水平均明显高于Ⅰ~Ⅱ期患者(P﹤0.01)。受试者工作特征(ROC)曲线分析结果显示,血清AFP联合CEA检测的约登指数为0.850,灵敏度为90.00%,特异度为95.00%,曲线下面积(AUC)为0.939。结论肝功能异常患者中AFP、CEA表达水平较高,其表达水平对临床诊断肝癌具有参考价值,两者联合检测肝癌具有较高的灵敏度和特异度。
Objective To explore the diagnostic value of serum alpha-fetal protein(AFP)combined with carcinoembryonic antigen(CEA)detection for liver cancer.Method A total of 160 patients with liver cancer were taken as the observation group,and 160 healthy patients at the same period were taken as the control group,the serum levels of AFP,CEA and liver function of the patients were detected.The serum levels of AFP and CEA of liver cancer patients with different liver function,tumor diameters and clinical stages were compared,and the diagnostic efficacy of serum AFP combined with CEA was analyzed.Result The levels of AFP and CEA in the observation group were higher than those in the control group(P<0.01);the levels of AFP and CEA of patients with abnormal liver function in the observation group were significantly higher than those with normal liver function(P<0.01).Serum AFP and CEA expression levels in patients with tumor diameter≥5 cm were significantly higher than those in patients with tumor diameter<5 cm(P<0.01).The serum levels of AFP and CEA expression in patients with tumor stage Ⅲ~Ⅳ were significantly higher than those in stage Ⅰ~Ⅱ patients(P<0.01).The results of receiver operating characteristic(ROC)curve analysis showed that the Youden index of serum AFP combined with CEA was 0.850,the sensitivity was 90.00%,the specificity was 95.00%,and the area under the curve(AUC)was 0.939.Conclusion The levels of AFP and CEA are highly expressed in patients with abnormal liver function,which has reference value for the clinical diagnosis of tumors in patients with liver cancer,AFP combined with CEA detection has a higher sensitivity and specificity.
