摘要
目的:探讨缓冲式膝关节支具对脑卒中后膝过伸的临床疗效.方法:将60例脑卒中后膝过伸患者随机分为观察组和对照组各30例.两组均进行常规康复治疗2周,且观察组3个月内于步行时佩戴缓冲式膝关节支具.分别于治疗前、3个月后行三维步态分析、下肢简化Fugl-Meyer评分(MBA)、"起立-行走"计时测试(TUGT)和改良Barthel指数(MBI)评定.结果:观察组膝过伸治疗有效率为86.67%,显著高于对照组的63.33%(P<0.05).3个月后,观察组步态参数(患侧膝过伸角度、步速、步幅、步频、健侧步长、健侧双支撑相)、步态不对称指数、下肢MBI、TUGT、MBI均较治疗前改善(P<0.05),且优于对照组(P<0.05),两组患者步宽、患侧步长、患侧支撑相、患侧双支撑相差异无显著性意义(P>0.05).结论:缓冲式膝关节支具可有效改善脑卒中后膝过伸患者患侧膝过伸角度和步态,提高患者下肢运动功能、步行功能及日常生活自理能力.
Objective:To investigate the clinical effect of cushioning knee brace on knee hyperextension after stroke.Methods:Sixty patients with knee hyperextension after stroke were randomly divided into observation group and control group with 30 cases each.Both groups received conventional rehabilitation treatment for 2 weeks,and the observation group wore a cushioning knee brace during walking within 3 months.Three-dimensional gait analysis,simplified Fugl-Meyer score(MBA),"up-to-walk"timing test(TUGT)and modified Barthel index(MBI)were performed before treatment and 3 months later.Results:The effective rate of knee hyperextension treatment in the observation group was 86.67%,which was significantly higher than 63.33%in the control group(P<0.05).After 3 months,the observation group's gait parameters(impact side knee hyperextension angle,stride speed,stride length,stride frequency,healthy side step length,healthy side double support phase),gait asymmetry index,lower limb MBI,TUGT,MBI was improved compared with before treatment(P<0.05),and better than the control group(P<0.05).There was no significant difference in step width,affected side step length,affected side support phase,and affected side double support phase between the two groups(P>0.05).Conclusion:The cushioning knee brace can effectively improve the knee hyperextension angle and gait of the affected side of the knee hyperextension patient after stroke,and improve the patient's lower limb motor function,walking function and daily life self-care ability.
