三维配准技术在全解剖氧化锆冠磨耗性能研究中的应用

Application of 3D superimposition methods on the investigation of wear of enamel antagonists against full contour zirconia crown

摘要 目的采用三维模型配准技术检测全解剖氧化锆冠对天然牙釉质的磨耗程度。方法选取上下颌第一磨牙需全冠修复的中年男性病例9例,对缺损牙采用精细抛光的全解剖氧化锆冠修复,修复后即刻(T0)及修复后12个月(T1),制取氧化锆冠对牙合局部牙列的石膏模型,模型三维扫描后获取牙列数字化模型。采用三维配准软件比较T0/T1期数字化模型牙齿牙合面的三维变化,计算差异的均方根值,并与自然磨耗的天然牙牙合面变化对比。结果氧化锆冠对牙合牙及对牙合牙近中、远中邻牙牙合面T0/T1期数字化模型配准均方根值分别为36.56μm,33.13μm,41.57μm,氧化锆冠对牙合牙的磨耗程度与邻牙无显著性差异,但近中、远中邻牙磨耗程度存在差异。结论精细抛光的氧化锆对天然牙釉质的磨耗与釉质自然磨耗接近,但牙位对磨耗程度有影响。 Objective To investigate the enamel wear caused by full contour zirconia crown under functional load using a 3D superimposition method.Methods Nine mid-aged male patients with first molar requiring full coverage crown were recruited,3 teeth in maxillary while 6 in mandible,each defect tooth received a fine polished full contour zirconia crown.The stone models of homolateral antagonists arch were made by Polyvinyl-siloxane impressions immediately(T0)and 12 months(T1)after crown cementation,then the stone models were scanned by a 3D scanner.Digital models were reconstructed and analysed using a 3D superimposition software to investigate the variance on the occlusal surface,the RMS of alighment was calculated for each separated tooth.Results The RMS of tooth antagonists against full contour zirconia crown and its mesio/distal adjacent one were36.56μm,33.13μm,41.57μm respectively.Significantly difference was found between the mesio and distal adjacent tooth of the crown antagonists,which shows no significantly difference with either of the former two.Conclusion The wear rate of fine polished zirconia surface is compared with the natural enamel,but tooth position has an effect on wear behavior.
