磁共振动态增强定量参数对克罗恩病病情的评估有效性分析

Analysis of the Validity of Quantitative Parameters of Dynamic Enhanced Magnetic Resonance in the Evaluation of Crohn's Disease

摘要 目的探讨磁共振动态(DCE-MRI)增强定量参数用于测评克罗恩病(CD)病情的效果。方法方便选择2018年4月—2020年6月前来该院就诊的60例CD患者,均采用MRI、DCE-MRI扫描及检测容积转运常数(K^(tans))值,采集入选患者的Harvey-Bradshow指数(HBI)及弥散系数(ADC)等资料,参照HBI水平分为静止组、轻-中度活动组与重度活动组,分别20例、25例、15例,比较3组ADC、K^(tans)检出值,解读活动期与慢性期CD患者MRI特征性表现及MRI、ADC、K^(tans)值的关系。结果静止组、轻-中度活动组与重度活动组ADC依次为(2.85±0.43)×10^(-3)、(1.43±0.22)×10^(-3)、(1.15±0.17)×10^(-3)mm^(2)/s,差异有统计学意义(F=178.110,P<0.001),以上3组K^(tans)值依次为(1.14±0.65)、(2.75±1.46)、(3.92±1.12)min-1,差异有统计学意义(F=25.550,P<0.001);纳入该研究的患者K^(tans)和HBI存在正相关性(r=0.359,P=0.013),ADC和HBI呈负相关(r=-0.648,P<0.001),血清ADC和K^(tans)呈负相关(r=-0.473,P=0.001)。结论MRI的定量参数K^(tans)值在呈现CD病患活动状态方面表现出良好效能,可作为CD临床诊断、治疗的依据。 Objective To discuss the effect of dynamic magnetic resonance(DCE-MRI)enhanced quantitative parameters for the evaluation of Crohn's disease(CD).Methods A total of 60 CD patients who came to the hospital from April 2018 to June 2020 were selected.MRI,DCE-MRI scans and volume transfer constant(K^(tans))values were used to collect Harvey-Bradshow index(HBI)and dispersion of selected patients.The coefficient(ADC)and other data were divided into static group,light-moderate activity group and severe activity group with reference to HBI level,with 20 cases,25 cases,and 15 cases respectively.Compared the detected values of ADC and K^(tans) in the three groups,and interpret the difference between the active period and the active period.The characteristics of MRI and the relationship between MRI,ADC and K^(tans) values in patients with chronic CD.Results The ADCs of the stationary group,light-moderate activity group and severe activity group were(2.85±0.43)×10^(-3) mm^(2)/s,(1.43±0.22)×10^(-3) mm^(2)/s,(1.15±0.17)×10^(-3) mm^(2)/s,the difference was statistically significant(F=178.110,P<0.001),the K^(tans) values of the above three groups were(1.14±0.65)min-1,(2.75±1.46)min-1,(3.92±1.12)min-1,the difference was statistically significant(F=25.550,P<0.001);K^(tans) and HBI of the patients included in this trial were positively correlated(r=0.359,P=0.013),ADC and HBI were negatively correlated(r=0.648,P<0.001),serum ADC and K^(tans) showed a negative correlation(r=-0.473,P=0.001).Conclusion The quantitative parameter K^(tans) value of MRI shows good efficacy in presenting the activity status of CD patients,which can be used as a basis for clinical diagnosis and treatment of CD.
