摘要
目的探讨综合救治方舱内行射频消融术的可行性。方法选取3只成年健康雄性比格犬为研究对象,常规麻醉固定后,经左右股静脉分别置入2根电极导管。利用射频消融术靶向消融右束支。观察实验动物术中及术后腔内心电生理图及右束支损伤情况。在方舱内开展射频消融术,治疗房室结折返性室上性心动过速患者1例。观察术中电生理仪和C型臂X线机工作情况、术中术后并发症发生情况,以及手术疗效。结果消融术前,3只实验动物心电生理腔内图可见窦性心律,波形正常。经右束支射频消融后,心电生理腔内图均出现典型完全性右束支传导阻滞图形。消融术后3 d,经过大体解剖及Lugol’s液染色观察,进一步确定3只实验动物均已通过消融达到完全性右束支阻滞。临床患者术后无不适主诉,无发热感染等症状。术后及3个月随访心电图均显示窦性心律,无术后并发症。结论综合救治方舱内行射频消融术具有较高安全性及可行性。
Objective To investigate the feasibility of radiofrequency ablation in the square cabin for field comprehensive treatment.Methods Three healthy adult beagles were selected as the research subjects.After routine anesthesia and fixation,two electrode catheters were implanted through the left and right femoral veins.The right bundle branch was targeted ablated by radiofrequency ablation,and the intraoperative and postoperative electrophysiological images and the injury of the right bundle branch were observed.A case of atrioventricular node reentrant supraventricular tachycardia was treated by radiofrequency ablation in the square cabin.The operation of electrophysiological apparatus and C-arm machine during operation,the incidence of intraoperative and postoperative complications and the surgical effect were observed.Results Before ablation,the electrophysiological endocardiogram of 3 experimental dogs showed sinus rhythm with normal waveform.After radiofrequency ablation of right bundle branch,typical complete right bundle branch block was found in all ECG intracavitary images.Three days after surgery,after gross anatomy and Lugol′s staining,it was further confirmed that all three dogs had achieved complete right bundle branch block by ablation.Clinical patients did not complain of discomfort,fever,infection and other symptoms.Postoperative and 3 months follow-up ECG showed sinus rhythm without postoperative complications.Conclusion Radiofrequency ablation in the cabin for comprehensive treatment has high safety and feasibility.
