摘要
目的探讨不同厂家、不同检测原理的血细胞分析仪对白细胞分类结果的一致性及与人工显微镜镜检结果的符合率。方法利用Sysmex XE-2100、XN-9000和Beckman Coulter LH750全自动血细胞分析仪和人工显微镜镜检分类方法对100例患者的EDTA-K2抗凝全血样本进行白细胞分类。结果针对白细胞计数在正常参考区间内的样本,Sysmex XE-2100和XN-9000的5种白细胞分类结果均具有较好的相关性(r>0.90),而Beckman Coulter LH750与Sysmex XN-9000之间嗜碱性粒细胞的相关性较差(r<0.60),其他4种白细胞的相关性均较好(r>0.90);针对白细胞计数不在正常参考区间内的样本,Sysmex XE-2100与XN-9000之间单核细胞的相关性较差(r<0.71),Beckman Coulter LH750与Sysmex XN-9000之间单核细胞(r<0.71)和嗜碱性粒细胞(r<0.65)的相关性较差。以人工显微镜镜检分类为金标准,Sysmex XE-2100和XN-9000的淋巴细胞、单核细胞的符合率较低(≤80%),Beckman Coulter LH750的淋巴细胞、单核细胞和嗜碱性粒细胞的符合率较低(≤80%)。结论3种类型的血细胞分析仪对于正常白细胞的识别具有良好的相关性和较高的符合率,而对于白细胞计数或分类异常(特别是淋巴细胞和单核细胞比例异常)的样本,相关性较差,符合率较低,需要建立适宜的复检规则,以免造成误诊、漏诊。
Objective The consistency of the classification results of leukocytes by blood cell analyzers based on different principles from different manufacturers and the conformity with artificial microscopic examination were investigated.Methods Leukocytes in EDTA-K2 anticoagulant whole blood samples from 100 patients were classified by Sysmex XE-2100,XN-9000 and Beckman Coulter LH750 automatic blood cell analyzers and artificial microscope method.Results In terms of the samples of which the leukocyte count was in the normal reference interval,the leukocyte classification results of Sysmex XE-2100 and XN-9000 showed a good correlation in five kinds of leukocytes(r>0.90),and Beckman Coulter LH750 showed a bad correlation with Sysmex XN-9000 in the classification of basophils(r<0.60),while good for the other four kinds of leukocytes.In terms of the samples of which the leukocyte count was not in the normal reference interval,Sysmex XE-2100 and XN-9000 showed a bad correlation in the classification of monocytes(r<0.71),while Beckman Coulter LH750 and Sysmex XN-9000 showed bad relations in the classification of monocytes(r<0.71)and basophils(r<0.65).Taken the results of artificial microscopic classification as the standard,the coincidence rate of Sysmex XE-2100 and XN-9000 in lymphocytes and monocytes were poor(≤80%),while as for Coulter LH750,the coincidence rate in lymphocytes,monocytes and basophils were poor(≤80%).Conclusion All the three types of blood cell analyzers have good correlation and coincidence rate for the recognition of normal leukocytes.But for the specimen with abnormal leukocyte count or classification(especially the proportion of lymphocytes and monocytes was abnormal),the correlation and coincidence rate were poor,so it is necessary to establish appropriate re-examination rules to avoid misdiagnosis and missed diagnosis.
