摘要
目的探讨磁共振弥散张量成像(DTI)联合弥散加权成像(DWI)在颅脑损伤患者病情评估中的应用价值。方法选取洛阳仁大医院2019年1月至2021年1月收治的106例颅脑损伤患者作为研究对象,根据格拉斯哥昏迷量表(GCS)评估患者病情严重程度,将其分为轻度损伤组、中度损伤组、重度损伤组。全部患者均接受DTI、DWI检查。比较3组患者DTI、DWI参数,即各向异性分数(FA)、表观弥散系数(ADC),分析DTI联合DWI对颅脑损伤患者病情的评估价值。结果106例颅脑损伤患者中轻度损伤62例(58.49%),中度损伤24例(22.64%),重度损伤20例(18.87%)。重度损伤组ADC值、FA值最低,其次为中度损伤组,轻度损伤组最高,差异有统计学意义(P<0.05)。受试者工作特征(ROC)曲线显示,ADC值、FA值评估颅脑损伤患者病情严重程度的曲线下面积(AUC)>0.70,当ADC截断值分别取6.795、6.445,FA截断值分别取0.625、0.600时,其对中度、重度颅脑损伤的评估价值最高。结论DTI联合DWI对颅脑损伤患者病情严重程度具有一定评估价值,临床可通过DTI、DWI的定量参数分析为临床颅脑损伤患者病情评估提供参考。
Objective To explore the application value of magnetic resonance diffusion tensor imaging(DTI)combined with diffusion weighted imaging(DWI)in the evaluation of patients with craniocerebral injury.Methods A total of 106 patients with craniocerebral injury admitted to Luoyang Renda Hospital from January 2019 to January 2021 were selected as the research objects.According to the Glasgow coma scale(GCS),the patients were divided into mild injury group,moderate injury group,and severe injury group.All patients underwent DTI and DWI examinations.The DTI and DWI parameters of the three groups of patients,namely fractional anisotropy(FA)and apparent diffusion coefficient(ADC)were compared.The evaluation value of DTI combined with DWI on the condition of patients with craniocerebral injury was analyzed.Results In 106 patients with craniocerebral injury,62 cases were mildly injured(58.49%),24 cases were moderately injured(22.64%),and 20 cases were severely injured(18.87%).The ADC and FA values of the severe injury group were the lowest,followed by the moderate injury group,and those of the mild injury group were the highest,with statistical differences(P<0.05).The receiver operating characteristic(ROC)curve showed that the area under the curve(AUC)of ADC and FA values for assessing the severity of craniocerebral injury patients were both greater than 0.70.When the ADC cut-off values were 6.795 and 6.445,and the FA cut-off values were 0.625 and 0.600,the evaluation value for moderate and severe craniocerebral injury was the highest.Conclusion DTI combined with DWI has a certain value in assessing the severity of patients with brain injury.
